Duties Description The candidate will clinically oversee a cluster of nurses and be responsible for multiple sites and may function as a RN1 as needed. Ensure nursing case management of multiple locations; caseload based on medical acuity. Oversee and supervise RNs and LPNs who will supervise the AMAP staff in providing Direct Support Assistants health care per the nurse Act Exemption. Ensure all medical follow-up is done timely. Responsible for in-service on nursing plans and other staff development and oversight as required, including medication administration, which are vital activities to regular operations. Engage in orientation and mentoring of new RNs and LPNs. Complete evaluations. Monitor/Complete nursing audit for the cluster, work closely with the Nursing Supervisor / Director of Nursing and Team to review findings, and develop a plan of correction. Monitors individuals’ medical/health needs and services. Interacts with primary medical specialists, hospitals, and various clinical staff as necessary. Advocate for individuals’ health care needs. Maintains ongoing communication with med liaison/designee. Provides support to site staff in maintaining health services for the individuals. Summary : The Home Health Aide (HHA), Personal Care Aid (PCA) provides direct care to residents following an individualized service plan, treats each resident with dignity and respect, recognizes individual needs and encourages independence. The HHA fosters a homelike atmosphere throughout the residence. Qualifications Maintains a current state license as a HHA or PCA Minimum of one-year experience in home health, assisted living or long-term care Possess the ability to read, write and communicate effectively in English May require lifting or bending. Must be able to lift 25-35 pounds Reports to: The HHA/PCA reports to the Resident Care Coordinator. Essential Duties and Responsibilities include but not limited to: Read the daily log when first reporting to work Assists residents with activities of daily living, including bathing, dressing, grooming, toileting, transferring and getting to and from activities and meals according to the individualized service plan. Allows and encourages residents to do as much of their own care as possible. Serves meals to residents in the dining room or apartments. Encourages residents to eat a proper diet and take adequate fluids while respecting the residents right to freedom of choice. Observes resident’s dietary needs and assists with feeding as necessary. Records and reports changes in residents eating habits to the supervisor. Initiates and participates in leisure activities provided for the residents as described in the activity calendar. Encourages residents to socialize and participate in planned activities or programs and to develop friendships with other residents. Daily documentation on all ADL sheets, incident reporting or changes in resident behavior, condition or personal needs and reports information to the supervisor in a timely manner. Obtains residents weights. Intermittent assist with emptying Foley catheters Intermittent assist with use of oxygen equipment. Changes dressings involving clean procedure to stable surface wounds. Assists with ostomy care when ostomy has achieved its normal function. Observes, records and reports all changes to the supervisor. All HHA’s must demonstrate competence in performing necessary skills and only perform those skills on the plan of care. Refer to policy on “HHA Scope of Tasks” Maintains a clean, safe, and orderly environment for the residents. Performs daily light housekeeping to the rooms which includes: picking up tissues, papers, cups etc. straightening bathroom. putting clothing away. hanging up the towels. making the bed. throwing out the garbage. closing the door. Following meals: HHA cleans tables and surrounding area including floors. residents need to be reminded to wash hands and toilet. HHA makes sure resident clothes are clean and presentable. HHA escorts residents to activities and participates. HHA does laundry as scheduled. HHA straightens up common areas as needed throughout the day. Follows proper procedures in emergency situations and responds promptly and positively to resident requests for assistance - including emergency pull cords, telephone calls and requests from family and friends. Throughout the shift: Incontinent residents are changed regularly and remain clean and dry. HHA is always aware of the whereabouts of their designated residents and notifies their supervisor immediately if unable to locate a resident. Encourages teamwork through cooperative interactions with co-workers. End of shift: HHA signs ADL sheets. HHA gives cross-over report to the following shift. HHA makes sure incontinent residents are clean and dry. Resident rooms are in order and no soiled products left in the garbage HHA signs daily assignment sheets and places in wellness office. Mandatory Staff Meetings/Trainings: All staff MUST attend monthly staff meetings and scheduled training to meet state regulations to maintain certification. These meetings are mandatory .

GENERAL JOB SUMMARY An exempt clinical position where the nurse practitioner (NP) is responsible for providing direct patient care. The NP participates as a leader of the skilled nursing facility (SNF) care team. Visits managed care and fee-for-service patients at skilled and long-term levels of care in designated SNFs facilities. Provides appropriate evidence-based geriatric medicine. Coordinates care with hospitalists, primary care physicians and care managers. Makes home visits as directed by the medical staff to meet patient needs and provide continuity of care. ESSENTIAL JOB FUNCTIONS Maintains privileges in multiple Nursing Homes as directed by ACA Maintains license and malpractice insurance Consults supervising attending as needed Documents patient visits electronically at least 90% of the time Participates in documentation and other quality improvement programs Available via phone weekdays 8am- 7pm and when on call. Will reviews, approves, and modifies admission orders Creates a detailed admit note for each admission within 24 hours of patient admission to SNF, including medication reconciliation Initiates/documents Advanced Directives Determines if Health Care Proxy status is correct and invoke if appropriate On weekends, takes call for admissions and see new patients within 24 hours of admission on a rotating basis with other practitioners in the program. Daily Visits Initiates and review orders, including medications, on a daily basis Reviews labs, radiology reports, and consults on all patients Talks to and examines each assigned skilled-level patient on daily rounds Monday through Friday Writes at least one daily progress note for each skilled patient Assess patient’s medical stability daily. Consults/coordinates with specialists as needed Addresses acute mental status changes via non-pharmacologic or pharmacologic measures, consultation or transfer Coordinates/assess rehab progress on a daily basis Discusses concerns with the patient, family, rehab, and case management. Educates patient and family members regarding acute and chronic illness management Attends family meetings as necessary Assists PCP’s that participate in SNF management Informs attending and/or ACA medical director of significant changes in medical condition Participates in weekly utilization meetings, collaborating with the SNF care team and ACA care managers Coordinates with PCP’s, Hospitalists, ACA Medical Directors and Case Managers Performs home visits on selected patients Addresses /coordinates any legal issues. Discharge Develops a discharge plan utilizing input from case management and rehab. Identify barriers to discharge Creates a detailed discharge summary for each admission on all patients, including medication reconciliation, and sends to the PCP at the time of SNF discharge Ensures that patients have all appropriate drug and DME prescriptions at discharge Coordinates visits with the PCP post-discharge Discharges summary to be sent to the PCP at discharge Updates all patients in Care Screen™ before discharge Coordinates transition from skilled to long term placement. Long-Term Care Assists case management in the evaluation of selected long term patients Follows “new” long term patients every 30 days Assists the attending physician with management for complex long-term patients Qualifications EDUCATION AND EXPERIENCE License to practice as a Registered Nurse and a certificate to practice as a Nurse Practitioner issued by the State Board of Registered Nursing.
