This role involves regular physical activity, including lifting patients over 200 lbs with assistance, transporting supplies and equipment up to 20 lbs for distances of 500 feet, and frequent standing, walking, sitting, stooping, and reaching. Responsibilities: Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and the Interdisciplinary Group (IDG) to develop, implement, and evaluate individualized plans of care in alignment with regulatory requirements and the Case Management Recipe. Maintains timely and accurate documentation, ensures care plans reflect current patient status, and communicates effectively with the care team to coordinate optimal use of resources, especially in complex or high-risk situations. Provides education and emotional support to patients and caregivers based on individual needs, and actively participates in on-call rotations, admissions, and care transitions as directed. If serving as a Care Manager, coordinates all aspects of patient care including scheduling, medication management, plan of care oversight, IDG collaboration, and supervision of LPNs, LVNs, and Hospice Aides; participates in IDG meetings and patient/family conferences. If serving in an On-Call capacity, responds to triage calls, performs visits and assessments, coordinates care with the Administrator On Call, and may attend IDG meetings and family conferences; also participates in training, committees, and other duties as assigned within licensure scope. The NP is responsible for using their licensure to obtain and maintain payer credentialing and reimbursement eligibility for the organization. Job Description: Job Responsibilities (listed in order of importance and/or time spent) Provide comprehensive, hands-on patient care including assessments, physical exams, and treatment of infusion therapy-related conditions. Prescribe and manage medications related to infusion care, in compliance with state regulations and collaborative practice agreements. Order and interpret labs, diagnostics, and therapy-specific tests. Oversee and perform IV, IM, and SC medication administration and monitor for side effects or adverse events. Create, document, and update individualized infusion care plans and maintain thorough, timely documentation in the EHR. Conduct initial patient evaluations for therapy appropriateness, including payer criteria and clinical suitability for infusion center setting. Maintain open, timely communication with medical directors, referring physicians, and interdisciplinary staff regarding care coordination. Identify urgent or emergent reactions during infusion, initiate appropriate interventions, and coordinate emergency transfers when necessary. Serve as a clinical leader within the infusion center team; provide guidance to infusion nurses and support staff on patient care and clinical protocols. Support patient education, counseling, and safe transition of care across healthcare settings. Participate in quality improvement initiatives and operational planning efforts across the infusion center. Participate in credentialing and contracting processes by maintaining all payer-required licenses and individual NPI enrollment linked to the organization’s Tax ID. Perform duties in compliance with HIPAA, OSHA, ACHC, and all applicable federal and state guidelines. Supervisory Responsibilities Does this position have supervisory responsibilities? Answer call lights promptly. Help check in all new residents and re-admissions, as well as assist in the transfer of residents to different rooms. Put resident's clothing away properly. Report any changes in resident's condition--e.g. eating habits, behavior, temperature, etc. to the charge nurse of the unit. Turn bedridden residents every two (2) hours or sooner as indicated. Wash, clean, and dry all incontinent residents. Assist the treatment nurse as assigned. Complete assignment report to charge nurse at the end of every shift. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Take vital signs and monthly weights (with assistance) and record results in EMR. Document the following in the EMR for all assigned residents: a) vital signs b) weight c) meal monitoring d) bowl and bladder e) restorative records Detect and report situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. SUMMARY: The Nurse is responsible for providing direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by nursing assistants. Receive & transcribe telephone orders from physicians & record on the Physician’s Order Form. Chart nurse’s notes in an informative & descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care. Fill out and complete accident/incident reports & submit to Director as required. Perform routine charting duties as required & in accordance with established charting & documentation policies & procedures. Prepare & administer medications as ordered by the physician. Develop & maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. Receive/give the nursing report upon reporting in and ending shift duty hours. Make rounds with physicians as necessary. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, etc., as required. Monitor your assigned personnel to ensure that they are following established safety regulations in the use of equipment and supplies. Maintain confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. Ensure that all nursing care is provided in privacy and that nursing service personnel knock before entering the resident’s room. Provide IV & IV site care as needed.
