Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. OR High school diploma or equivalent and completion of one of the following programs: medical assistant, emergency medical technician, nurse assistant, licensed practical nurse, armed services medic, or patient care technician.
