POSITION OVERVIEW: The Director of Nursing (DON) is a key member of the agency’s leadership team and is responsible for the overall clinical and operational management of the Nursing Department. This role ensures the delivery of safe, high‑quality, person-centered nursing services across OPWDD and DOH certified ICF programs, and provides strategic direction that supports optimal health outcomes, regulatory compliance, and organizational excellence. RN Coordinator

Day Shift - 7.5 Hours (United States of America) The RN Coordinator is responsible for the continuity of patient care across the Amboy Medical Practice and supports the clinical and quality performance goals of the practice. The RN Care Coordinator routinely assesses patients for readiness to learn, barriers to care, social determinants of health and other psychosocial behaviors that impact care. The RN Care Coordinator is a resource to providers and clinical staff and ensures that the practice understands and meets the needs of its patients through pre-visit planning, daily huddles, referral and test tracking a care coordination services.
