The Family Medicine team is uniquely trained to care for patients as a whole person, regardless of age or sex. The cornerstone of Family Medicine is an ongoing, personal relationship between caregivers and patients to achieve the best possible health outcomes. Caregivers and patients work together to achieve the best possible health outcomes. A caregiver in this position works Monday through Friday from 8:00am to 5:30pm. Once a week there will be coverage needed until 7:30pm. Rotations include 1 weekend a month from 8:00am to 12:00pm. A caregiver who excels in this role will: Assist with performing screenings and treatments by gathering and updating information related to health maintenance, identifying medication changes, updating allergy information, collecting specimens, performing tests and documenting patient care. Monitor and communicate changes in patient condition. Maintain equipment, medical supplies and examination areas. Perform administrative/clerical duties, such as filing, reception, scheduling, data entry and patient registration. Minimum qualifications for the ideal future caregiver include: Graduate from an approved medical assisting program OR military training as a hospital Corpsman (HM), Combat Medic (68W) or Medical Service Technician (4N0X1) Completion of a clinical externship OR extensive clinical experience Basic Life Support (BLS) certification through the American Heart Association (AHA). How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. Key Responsibilities Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed. Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures. Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines. Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel. Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care. Additional Details We are looking for a part time LPN to join our clinical team at Cuyahoga Juvenile Detention Center. This role is 24 hours a week. Week One: Sunday, Monday Week Two Monday, Saturday Qualifications & Requirements Education Graduate from an accredited school of nursing. Experience Prefer a minimum of one (1) year clinic experience Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
