Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Qualifications Required: Graduate of a state-approved School of Practical Nursing. Currently licensed as License Practical Nurse by the State of Ohio through the Ohio Board of Nursing. Current AHA Basic Life Support for the healthcare provider certification or obtains within 90 days of hire. 2 years of healthcare experience as an LPN, preferably in primary care, outpatient care, or care management. EMR knowledge and experience. Possesses a strong work ethic and ability to work independently and as a member of a multidisciplinary team. Displays a passion and strong commitment to delivering excellent customer service, with a preference for doing so in a healthcare setting. Knowledge of basic medical procedures and medical terminology. Excellent written, verbal, and interpersonal communication skills, and able to use those with a diverse patient population. Demonstrates strong analytical, data management and computer skills, including related to health-related data and data systems. Proficiency with the Microsoft Office suite of products. Detail-oriented and demonstrates strong leadership, organizational and time management skills. Demonstrates solid judgement, flexibility, initiative, and problem-solving abilities. Willing and able to work a flexible schedule, including some evenings or weekends. Access to reliable transportation to travel between locations both within and outside The MetroHealth System based on operational needs. Preferred Experience with the Epic electronic health record system. Fluency in writing, speaking, and interpreting both English and another foreign language. Physical Requirements: May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. Ability to lift 250-300 pounds with assistance. Ability to push/pull up to 500 pounds with assistance. Requires some exposure to communicable diseases or body fluids.
