Summary: The Advanced Clinical Educator is a master’s prepared clinician who serves as a role model, consultant, change agent and facilitator in assessing clinical learning needs and in planning, implementing, and evaluating educational activities for the clinical staff. The Advanced Clinical Educator maintains exemplary professional interpersonal communication skills and displays advanced analytical ability, logical thinking, proficiency in concept formation and conceptualization of complex theories, as well as creativity and ability to analyze, anticipate, and recognize opportunities and potential problems. Position Responsibilities: • Creates an innovative, adaptive and integrated learning culture for clinical and ancillary staff to support the five points of the star and the strategic imperatives. Anticipates and forecasts trends. • Uses organizational strategic plan and goals to determine educational priorities. • Works collaboratively with staff members and management system-wide to assess learner characteristics, including learning styles and learning needs for program development at the system level. • Applies clinical and learning theoretical and conceptual foundations as a basis for developing professional development programs that are culturally relevant and that incorporate concepts of multicultural and multigenerational education. • Implements a variety of teaching strategies tailored to the learners’ characteristics, learning needs, cultural perspectives, and outcome objectives. • Uses outcome measurements and outcome evaluation methods to align with the strategic imperatives and demonstration return on learning investment. • Collaborates with other members of the leadership team to assure competency assessment plans for orientation and annually. • Participates in and incorporates research and/or evidenced based practice as an advisor, investigator, collaborator, translator, integrator, or evaluator. • Consistently uses planning methodologies when implementing, facilitating, and evaluating various system-wide projects. Annual Salary: $104,613 - $167,938 The actual salary/rate will vary based on applicant’s experience as well as internal equity and alignment with market data. Schedule and participate in interdisciplinary team meetings. Advocate for members to ensure safe and appropriate transitions of care. Develop individualized care plans and authorize services in a cost‑effective manner. Document accurate and timely information in the electronic health record. Use sound clinical judgment and problem‑solving skills in daily decision‑making. Mentor new hires once proficiency is achieved. Successfully complete company‑sponsored NJ Choice Certification (required score of 80% or higher for continued employment). Member assignments may include pediatric populations and medically complex cases. Required Qualifications Active, unrestricted RN license in New Jersey Comfort conducting field‑based home visits across all socioeconomic levels and community settings , including any neighborhood or environment, and with members in a wide range of living conditions or circumstances. 3+ years of clinical practice experience 3+ years of experience in home health, hospice, or a similar setting Residency in Salem County preferred (Gloucester or Cumberland County may be considered) Willing and able to travel 50–75% within assigned counties Successful completion of the NJ Choice Certification , including: Minimum exam score of 80% Completion of state training modules Field mentoring component Preferred Qualifications Case management and/or discharge planning experience Managed care experience Crisis intervention skills Education Associate’s degree in Nursing (minimum requirement) Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $66,575.00 - $142,576.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
