Plans and develops: Individualized outcomes-based care plans incorporating the patient and family’s readiness and barrier to learning; Orchestrates and validates a plan of care for each shift; Initiates interdisciplinary patient/family meetings to facilitate communication regarding the plan of care; Communicates and coordinates the patients discharge plan utilizing unit-based and community resources to ensure a seamless transition for the patient and their family; Ensures the plan of care utilizes principles and concepts of project, organizational or systems management. 4. We are hiring a full-time MDS Coordinator to join our dynamic care team at Southeast Rehabilitation & Skilled Care Center in North Easton! Can be LPN or RN. Weekly pay ranges from $40 to $48 an hour, depending on license and experience. Southeast is a 171-bed skilled nursing facility. As the MDS Coordinator, you will play a crucial role in planning, organizing, and directing the MDS process, ensuring that our residents receive high-quality care and services. Experience & Education: Must possess, as a minimum, a Nursing Degree from an accredited school of nursing, college or university. Must possess a current, unencumbered license to practice as an RN or LPN in Massachusetts. Must have at least one year of nursing experience.
