Formulates individualized plans of care based on assessment findings and interpreting trends in patient populations to achieve best practice. Conducts all patient care in a patient and family centered manner, integrating knowledge, skills and experiences to meet the needs of patients and families throughout the continuum of care. Advocates for patients' rights from the patient and family perspective and provides counsel while acting as a resource to resolve issues. Accesses resources from all disciplines to achieve desired patient outcomes and coordinates healthcare and alternate services for patient/family transition planning. Documents all patient care activities, as per documentation standards, and assists healthcare team members with documentation. Evaluates the process to achieve outcomes, anticipates patient variances and makes revisions to plans of care. Makes clinical decisions based on experience, patient population data and intuition while promoting the development of clinical decision making of all team members. Minimum Qualifications Education: BSN or AD. If RN has an AD, within 2 years from date of hire, they must meet with their nurse leader and conduct the following: 1.) Identify which accredited school they plan to attend 2.) Provide a written plan with anticipated BSN completion date 3.) Submit a review of transcripts from the school indicating the required pre-requisites and timeline for taking the courses 4.) Complete BSN within 5 years of start date Experience: 1 year of experience as a Registered Nurse, for Critical Care units, 1 year of experience as a Registered Nurse in a critical care setting or completion of a critical care fellowship. Certification/Licensure: BLS & ACLS from American Heart Association; Licensed or eligible for licensure in the Commonwealth of Virginia as a Registered Nurse or an active multi-state Registered Nurse license Preferred Qualifications Experience: Pulmonary or Cardiac experience is preferred, but not required. About this Job: General Summary of Position The Medical Assistant performs clinical and administrative duties in an ambulatory care setting under the supervision of a physician/provider and/or a registered nurse. Under onsite provider supervision may insert intravenous catheter for physician initiated IV fluid therapy or administration of intravenous medications. Under direct onsite physician supervision may insert remove and monitor urinary catheter for procedures and prepare and administer oral drugs and injections limited to intradermal subcutaneous and intramuscular. May perform phlebotomy. Utilizes the EMR to propose patient medication refills for provider approval. Completes requisitions for tests (if applicable). Contacts insurance companies for Pre-authorizations for patient medications and specialty referrals including insurance authorization. Floats to other sites/locations within the system as appropriate and performs all duties at multiple sites. Summary/Objective In keeping with our organization’s goal of improving the lives of the Residents we serve, the Registered Nurse (R.N.)/Licensed Practical Nurse (L.P.N.) plays a critical role in providing superior customer service and nursing care to all Residents and residents. The R.N./L.P.N. provides supervision of staff and will safeguard the health, safety, and welfare of all Residents/residents under their care by following applicable laws, regulations, and established nursing policies and procedures. Review daily the documentation of the dispensing of the controlled substances and narcotics. 7. Place pharmacy orders, for and administer all newly prescribed medications and document. 8. Carry out direct contemporaneous charting in your shift. Chart detailed monthly evaluations of each Resident that reflect his/her condition and progress. 9. Responsible for all nursing care of assigned Residents while on duty. Must notify appropriate persons if there is any significant change in a Resident’s condition or any transfer to a hospital. 10. Ensure that Resident care plans are being followed and asses each Resident’s status in accord with their care plan. 11. Round with physicians when needed. 12. RN’s to assess residents for change in clinical condition and prn. 13. LPN’s to monitor residents for change in clinical condition and prn. 14. Administer or supervise all treatments prescribed by physicians including but not limited to pressure ulcer care, Foley catheter care. 15. Supervise serving of prescribed diets and fluids intake and immediately report any issues to the physician and DON/ADON. 16. Must be knowledgeable of individual resident focused care plans and support the care planning process by reporting specific information and observations of the Resident’s needs, preferences and report any behavioral changes. 17. Provide wound care when needed. 18. Completes medical records documenting care provided and other information in accordance with nursing policies while maintaining strict confidentiality. 19. Assure that established infection control and standard precaution practices are maintained when providing care. Follow established safety precautions when preforming tasks and using equipment and supplies. 20. Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons. 21.
