LPN documents nursing history and physical assessment for assigned patients upon admission; Handles resident, family, or physician complaints/concerns, referring issues to CSD as necessary. Respects advance directives and assists residents to a dignified death; and provides emotional and spiritual support to family and other staff as needed. Responds to life threatening situations based upon nursing standards, policies, procedures, and protocol. LPN participates in department and/or unit performance improvement activities. § Attends in-service training programs for department specific and other relevant programs, i.e., safety, infection control, orientation, etc., as scheduled. § Is aware of and carries out general community and specific departmental safety policies; always reports hazards/safety issues promptly. § Is aware of and practices principles of infection control; reports infection control issues promptly. Makes occupied and unoccupied beds, changes bed linens, and straightens rooms, closet, and drawers. Notifies housekeeping if mattress in need of cleaning or replacement. Answers call lights promptly, answer telephone calls as needed and run errands as requested. Obtains blood pressure, temperature, pulse, respiration, heights, and weights as needed. Does complete check of skin condition when dressing, undressing, turning/repositioning, etc. Reports any skin abnormalities to the Staff Nurse. Observes customer’s physical condition, attitude, reactions, appetite, etc., and reports any changes and/or unusual findings to the Nurse/RCC so care plan can be updated. Performs personal services for customer’s (i.e., reading, writing letters, etc.) as time allows. Encourages customers to help themselves and promotes self-care in aiding rehabilitation. Assists with care of dying and expired customers. Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident’s eating habits to the Staff Nurse. Keeps customers water pitchers clean and filled with fresh water on each shift unless contraindicated. Serves between meals and bedtime snacks. Wears transfer belt at all times and utilizes as needed. Keeps the nurses’ call system within easy reach of the resident. Reports all hazardous equipment and conditions to the Staff Nurse immediately. Commit to obtaining at least 12 hours of continual education annually for self, including the required 16 hours of orientation training and any other ongoing dementia care specific trainings for direct care staff. Assist in oversight of the Memory Care Aides and Certified Nursing Assistants (CNAs), including supervision and assistance with ADL care, transportation, and non-scheduled activities by staff. Become knowledgeable of each individual resident’s background, life history, disease, medical and behavioral care needs, and preferences in detail. Utilize any communication devices for residents, such as language services, communication boards, assistive devices, etc. Accept education for utilizing these mechanisms to provide ample communication. Assist in providing a therapeutic, person-centered memory care activity program that constantly strives to enhance the quality of life of each resident, meets the Memory Care Program Mission and Philosophy, and upholds the Memory Care policies and procedures and all state and federal standards; commit in providing non-scheduled activities through the use of my talents and training. Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings. Ability to engage and communicate effectively with elderly and chronically ill patients and their families. Ability to maximize onsite time to develop robust caseloads Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.
