The In-Home Services RN (Registered Nurse) provides skilled nursing care in accordance with InnovAge policies and all applicable regulations to participants and performs related administrative tasks for InnovAge. Job Overview The Neonatal Nurse Practitioner (NNP) provides expert Family-Centered Care across the wellness-illness continuum, providing primary, acute, chronic and critical care to infants and toddlers up to age 2 years. In collaboration with other health care providers the NNP will provide initial, ongoing and comprehensive care, including taking complete histories; providing physical examinations and diagnosing, treating and managing patients with acute and chronic illnesses and diseases. This includes ordering, performing, supervising, and interpreting laboratory and imaging studies; prescribing medication and durable medical equipment; and making appropriate referrals for patients and families. Minimum Qualifications Degrees Bachelor of Science in Nursing & Master’s Degree NNP Experience Three years’ combined experience as RN or NNP in a neonatal setting. High risk neonatal setting experience preferred. Licenses & Certifications Registered Nurse License (RN); Advanced Practice Registered Nurse (APRN) License; Prescriptive Authority License; Board Certification as a NNP; Credentialing must be completed by the Medical Staff Office (MSO) within 3 months of employment BLS/CPR from the American Heart Association; Neonatal Resuscitation Program (NRP) Certificate and optional Pediatric Advanced Life Support (PALS). The Colorado Coalition for the Homeless seeks a Micro Community Nurse who works collaboratively within an Integrated Health Care for the Homeless and supportive housing teams. The RN Case manager in this position is responsible for performing both independent and delegated duties in the provision of providing comprehensive health care to assigned patients within the Colorado Coalition for the Homeless (CCH). The Nurse case manager primarily provides services in the community within the framework of the nursing scope, professional nursing ethics, and state and federal laws and regulations. This is a field-based position with most of the services provided to patients in settings outside primary health center sites. The nurse case manager collaborates with other integrated health and supportive housing service team members to deliver care to clients living on the streets and in supportive housing communities.
