About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Responsibilities Assists providers, nursing and other health care professionals with procedures and the intake and depart processes. Demonstrates skills necessary to assist with casting, splinting, and dressing application, removal and cleaning. Safely removes casts, cleans patient, covers affected limb and instructs family on skin care. Accurately documents information in the Electronic Medical Record (EMR) Responsible for prepping clinic to be prepared for that day's clinic to include monitoring supplies and equipment and ordering supplies Utilizes good clinical judgement to identify problems and interventions with patients and families and referral to RN as needed This is not an all-inclusive list of this job's responsibilities.
