The Registered Nurse Case Manager is responsible for admitting patients for home care services, developing the patient's plan of care, coordinating with the patient's provider and other disciplines for the provision of services, and ensuring the safe delivery of patient care. Collaborates with providers, other disciplines, and communicates all changes promptly. 3. Assesses payment sources and is responsible for ensuring ongoing authorizations as necessary. 4. Educates and instructs patients/caregivers in their ongoing medical maintenance. Begins discharge planning on admission. 5. Completes all documentation within 24 hours. Must include changes in condition, follow-up with provider or other disciplines with supporting documentation. 6. Evaluation of patient progress and prompt action 7. Oversight of LPNs and Home Health Aides. 8. Schedules patients independently according to the individual plan of care. 9. Attends all staff meetings, inservices, other required meetings. Compliance with all mandatory requirements. 10. Follows policies and procedures. 11. Thorough and accurate OASIS documentation. 12. Coordinates and manages the care of patients receiving services in their home. 12.1 Participates in case management conferences and documents per guidelines. 13. Participates in after-hours, weekend, and holiday coverage as needed. 14. May perform other duties as assigned. Qualifications EDUCATION Nursing Diploma or Associates Degree in Nursing. Bachelors in Nursing preferred. EXPERIENCE Minimum two (2) years of nursing experience in an acute care setting. Previous home care experience is desirable but not required. LICENSURE Registered Nurse with a valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire. SPECIAL SKILLS Excellent clinical knowledge, verbal/written skills, and organizational skills. Must be able to work independently. Able to competently assess patients' needs and follow through accordingly. Strong ability to work with providers and members of the multidisciplinary team. PHYSICAL DEMAND Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Connecticut shoreline (Groton-New Haven) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow. The Standards of Practice describe a competent level of nursing care as demonstrated by utilizing the nursing process components of assessment, diagnosis, outcomes identification, planning, implementation and evaluation. Assessment: The registered nurse collects pertinent data and information relative to the healthcare consumer's health or situation Diagnosis: The RN analyzes the assessment data to determine the actual and potential diagnoses or the issues. Outcomes Identification: The RN identifies expected outcomes for a plan individualized to the healthcare consumer or the situation. Planning: The RN develops and modifies a plan of care that prescribes strategies and alternatives to attain expected outcomes. Implementation: The RN implements the identified plan. Coordination of Care: The RN coordinates care delivery and transitions in care. Health Teaching and Health Promotion: The RN employs strategies to promote health and a safe environment. Evaluation: The RN evaluates progress toward attainment of outcomes. 2. STANDARDS OF PROFESSIONAL PERFORMANCE The ANA Standards of Professional Performance (2015) describe a competent level of behavior in the professional role, including activities related to: ethics, culturally congruent practice, communication, collaboration, leadership, education, evidence-based practice and research, quality of practice professional practice evaluation, resource utilization and environmental health. The RN is expected to engage in professional role activities, including leadership, appropriate to their education and position. The RN is accountable to for their professional actions to themselves, their healthcare consumers, their peers and ultimately to society. Ethics: The RN practices ethically. Culturally congruent practice: The RN practices in a manner that is congruent with cultural diversity and inclusion principles. Communication: The RN communicates effectively in all areas of practice Collaboration: The RN collaborates with healthcare consumer and other key stakeholders in the conduct of nursing practice. Leadership: The RN leads within the professional practice setting and the profession. Education: the RN seeks knowledge and competence that reflects current nursing practice and promotes futuristic thinking. Evidence-Based Practice and Research: The RN integrates evidence and research findings into practice Quality of Practice: The RN contributes to quality nursing practice. Professional Practice Evaluation: The RN evaluates owns' and others' nursing practice. Resource Utilization: The RN utilizes appropriate resources to plan provide and sustain evidence-based nursing services that are safe, effective and fiscally responsible Environmental Health: The RN practices in an environmentally safe and healthy manner. 3. May assist and/or perform chaperone duties for patient medical procedures as needed. Qualifications EDUCATION Graduation from an accredited School of Nursing. EXPERIENCE Registered Nurse is the foundational job description for all RNs. LICENSURE Licensed and a currently registered nurse in the State of employment. PHYSICAL DEMAND Requires prolonged standing; walking; use of sight, hearing, and touch. May be exposed to infectious diseases. Will be exposed to physical, mental and emotional illness as well as end of life conditions and death. Moderate stress and risk of injury from patient care. Standing/walking for more than half of the day. Frequently bending, reaching, pushing, pulling, twisting and lifting.
