MDS Coordinator The primary role of the MDS Coordinator is to coordinate and guide the overall process and tracking of all Medicare/Medicaid case-mix documents, and care assessments in order to assure appropriate reimbursement for services. The position is also responsible for completing concurrent MDS to assure achievement of maximum allowable RUG categories, as well as integrating information from Nursing, Dietary, Social Services, and Rehabilitation departments for appropriate reimbursement. Responsibilities: Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process. Coordinates and facilitates care conferences with the interdisciplinary team. We are seeking experienced Physician Assistants or Nurse Practitioners who have urgent care, emergency room experience, or high acute family medicine experience treating both adult and pediatric patients, to join our welcoming and diverse community of providers in the surrounding region . Job Summary Urgent care Advanced Practitioners (Physician Assistant/Nurse Practitioner) provide a wide variety of clinical services including treatment of acute episodic conditions such as sore throats, colds, flu-like symptoms, cuts, sprains and minor infections. Functioning in lieu of or alongside of a physician, it is essential for an urgent care Advanced Practitioner to have excellent assessment skills in order to recognize more serious conditions and direct the patient to the emergency room for advanced care when necessary. Per Diem employes must be available to work 3 days in every 28 days block including 1 weekend shift. Job Requirements Education Bachelor's Degree required for Physician Assistants Master's Degree required for Nurse Practitioners Work Experience One (1) year of full-time experience in Emergency Medicine or One (1) year of Urgent Care setting experience required . Required Licenses/Certifications National Certification Active state medical licensure Additional Knowledge, Skills and Abilities Work experience in an Urgent Care or Emergency Room setting preferred Applicable office procedures including but not limited to suturing, splinting, casting etc. Preliminary reads on EKGs and X-rays required . Experience treating patients ages 6 months and up in an acute care setting required . Additional Knowledge, Skills, and Abilities Preferred Knowledge and experience with Electronic Medical Records Ability to be attentive with strong listening skills Strong communication and customer service skills Essential Functions Essential functions are those tasks, duties and responsibilities that comprise the means of accomplishing the job’s purpose and objectives .
