Job Summary: We're looking for a part-time Nurse Practitioner or Physician Assistant with a background in Geriatrics or Internal Medicine to deliver high-quality preventive care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. As an integral part of our clinical team, you'll be responsible for building strong relationships with facilities, maintaining a patient-centered approach, and being a collaborative team player.
