Job Summary: The Intake Liaison Nurse acts as a professional resource to the facility and manages/facilitates the safe and timely transfer/referral of patients from an in or outpatient facility to the care of Personal Touch Home Aides. Key Responsibilities: Evaluates patients referred for home care to determine appropriateness for admission Accepts or rejects patients based on Agency's admission criteria. Recommends alternative plans for patients that do not meet Agency's admission criteria. Establishes and promotes an ongoing communication system with facility personnel including, but not limited to, physicians, discharge planners, social workers, registered nurses, utilization review, dietary and administration, while establishing a coordinated home care plan prior to discharge. Assesses the appropriateness of the requested services, medical supplies and appliances, based on goals mutually acceptable to the patient/ family. Notifies appropriate personnel at the Agency of patient referral, provides intake data, essential background information, and signed medical orders. Assures that Agency is ready to meet the patient's needs at the time of discharge by collaborating with Agency personnel on a regular basis. Explains the Agency's policies and services to patient and responsible family members following home care assessment and referral. Acts as a liaison with patient/families during the transitional period between the time the patient is discharged until admission visit. Determines financial coverage for requested home health services and informs patient/family of source(s) and extent of coverage and interprets reimbursement/coverage issues to facility staff. Develops, implements and evaluates systems to hand the management of referrals between source of referral and Agency to promote continuity of care. Provides an educational and consultative resource to the facility concerning appropriate home care services and community resources. Interprets governmental regulations regarding home care. Provides statistical data as required. Establishes and promotes an ongoing collaborative relationship between the Agency and facility to ensure the coordination of appropriate referrals. Acts as an Agency representative at facility based and community programs and functions. Assumed responsibility for professional growth to optimize clinical and managerial skills.
