Hunterdon Care Center is seeking a skilled, detail-oriented MDS Coordinator to join our team. The ideal candidate will possess excellent communication and organizational skills, collaborate effectively with the Interdisciplinary Team, and ensure accurate, timely completion of all MDS assessments and related documentation. RNs with Nursing home experience are Welcome! Currently hiring for Full Time Employment Key Responsibilities: Schedule and ensure timely completion of MDS assessments using PCC software . Maintain compliance with all state and federal regulatory requirements Ensure accurate and timely electronic submission of MDS to the state Attend and coordinate weekly utilization review meetings, collaborating with therapy and care team members Participate in the development, updating, and completion of individualized care plans Oversee and support facility staff in the MDS process as needed Monitor and analyze Quality Measure (QM) reports to identify trends, opportunities for improvement, and areas requiring corrective actio Participate in facility meetings, quality assurance initiatives, and interdisciplinary care conferences as required. Maintain and update appropriate ICD-10 coding for all residents Utilize Point Click Care (PCC) software to manage assessment schedules, documentation, care planning, and submission processes. Serenity Hospice is looking to add a dedicated and organized Hospice RN Team Lead to our Central team. The Hospice RN Team Lead is responsible for providing leadership and direction to the designated team of nurses to ensure the highest standards of patient care. The Team Lead is responsible for developing and implementing patient care plans, assessing patient needs, and ensuring that all care is delivered in accordance with established protocols and best practices. Responsibilities: Participates in the coordination and planning of the delivery of quality, cost-effective hospice care. Evaluates patient services provided by the interdisciplinary team. Maintains working knowledge of hospice state licensure, federal and accreditation requirements. Monitor services and documentation to meet hospice policies and procedures, state, federal, and accreditation guidelines. Serve as a liaison to the Administrator regarding clinical challenges and progress in patient care. Supervise and participate in the provision of care provided, orientation, scheduling, education, and support of all caregivers on the hospice interdisciplinary team, including any contracted staff. Be available on-call if needed. Provide support for on-call staff. Schedule and run meetings for the interdisciplinary team (IDT). Develop physician relationships to familiarize them with hospice care. Act as a liaison between IDT members, patients, and families. Contribute to effective patient and customer relationships by assisting patients, families, physicians, and staff to resolve expressed concerns. Facilitate interdepartmental communications with co-workers and staff. Perform other duties as needed. Requirements: Current NJ RN License in good standing 2 Years of Hospice experience required Prior progressive management responsibilities Strong organizational skills. Valid Drivers License with Auto Insurance. Reliable transportation and willing to travel. Proficient in Microsoft Word and Excel. Strong computer skills and ability to learn new software.
