Job Summary The Psychiatric Nurse Practitioner will provide general outpatient psychiatric services to members of our onsite community, including psychiatric intakes and evaluations for acute and chronic conditions along with related treatments, medication evaluations and treatment option planning. This role will work closely with our primary and ancillary care teams, including other behavioral health providers, as part of our larger comprehensive primary health model. Job Responsibilities Assess and treat acute and chronic psychiatric conditions through a combination of psychotherapy, psychoanalysis and medication management; discussing progress toward goals with patients, providing instructions, monitoring effect of medication(s), with referral for inpatient services as needed. Develop individualized care and treatment plans by determining the nature and extent of cognitive, emotional, developmental, social, and behavioral disorders, and establishing treatment goals and methodologies, collaborating with behavioral health providers as appropriate. Engage in regular discussions with patients to change behavioral patterns, explore past experiences, and/or conduct group and family therapy sessions. Provides treatment for acute and chronic psychiatric conditions Completes medication reviews and prescribes medications Advise or inform guardians, relatives, or significant others of patients' conditions or treatment, as appropriate. Provides evidence-based, timely, compassionate care Works with a multidisciplinary medical team, including primary care providers, health coaches and other behavioral health specialists Complete thorough documentation using EMR and other integrated tools and applications, collecting medical information from patients, family members, and other medical professionals. Actively participates in quality improvement programs, as well as in multidisciplinary quality assurance programs Collaborates with the management team as needed to ensure improvement of patient care Cooperates with clinical outcome reviews and measurements Comply with all Crossover Health policies, procedures, and protocols Performs other duties as assigned. Required Qualifications NP degree from an accredited program Active Washington State Medical License Completion of accredited Psychiatric-Mental Health Nurse Practitioner (PMHNP) Post-Master’s certificate program American Nurses Credentialing Center (ANCC) Psychiatric-Mental Health Nurse Practitioner (PMHNP) certification. Minimum 5 years of clinical experience serving patients across the care continuum of psychiatric care Current BLS (Basic Life Support) certification Preferred Qualifications Experience in an outpatient setting, specifically within an integrated primary care model Excellent communication skills both verbal and written, including presentation and consultation skills. Experience with telehealth or virtual care offerings, including aptitude for associated technology. Registered Nurse-Heart Failure/Heart Transplant Coordinator 0.9FTE Days 9 hour shifts, 8:30AM Start Relocation Assistance Available. Support the care of children with heart failure, those listed for transplant, coordinating organ procurement and transplantation, and managing the care of patients post transplant. As an integral member of the Health Care Team, the RN Transplant Coordinator functions within the Scope of Practice as defined by Washington State, the ANA Scope and Standards for Pediatric Nursing Practice, and adheres to Seattle Children's policies/procedures and guidelines of care. Responsible for providing safe, family centered patient care. Using the Nursing Care Process, the RN is responsible for assessing, planning, implementing and evaluating patient care in a collaborative method with the patient and patient's family, significant others, and members of the health care team. Coordinates clinical aspects of solid organ transplant patient care beginning with the evaluation for transplantation and continuing through and after transplantation. May be required to provide supervision and delegation of tasks to Nurse Technicians, Licensed Practical Nurses, Certified Medical Assistants, etc.., as defined by Washington State Nursing Commission's applicable WAC. Assists in meeting Seattle Children's strategic goals by incorporating the initiatives to support the strategic plans into the daily work, i.e. Continuous Process Improvement (CPI), ART, etc. In accordance with House Bill 1155 (effective January 1, 2020), the following areas have clinical unpredictable needs and as a result will have a call obligation within their nursing positions: Apheresis, Cardiac Cath Lab, Dialysis, ECMO, Home Care Services, Imaging Services (including GI Procedures), Perioperative Services, and Transplant Coordinators. Required Education and Experience Graduate of an accredited school of nursing; BSN preferred. Per the driver's license requirement noted below, must have a driving record acceptable to Risk Management. Required Credentials Current Washington State RN License. Current Basic Life Support (BLS) for Healthcare Providers is required for all positions. Pediatric Emergency Assessment, Recognition and Stabilization (PEARS), and Pediatric Advanced Life Support (PALS) will be accepted in lieu of BLS for Healthcare Providers. ACLS will not be accepted as a substitute to the requirements listed above. Employees are responsible for any costs (including time away from work) associated with obtaining PALS or PEARS certification if it is not a requirement of the position. Nurses working in the following areas are required to successfully obtain PALS within 3 months of employment, and subsequently maintain at all times: Cardiac Intensive Care Unit.
