Job Opportunity: MDS Care Plan Coordinator (RN) Schedule: Part-time Please note: Candidates without prior experience in MDS (minimum data set) will not be considered for this position. Job Description: MDS Care Plan Coordinator leads the MDS department, assists in coordinating discharge planning as well as assessing short term residents. The ideal candidate will possess skills to maximize reimbursement as well as ensure Medicare compliance. The MDS Care Plan Coordinator may be responsible for managing short term managed care cases. The MDS Care Plan Coordinator gathers information, assesses needs, establishes reasonable goals, provides interventions and incorporates within an organized, concise, functional care plan. Coordinates completion of comprehensive assessment by interdisciplinary team and includes recommendations in the written care plan for each resident. Each plan must identify all relevant issues for the care of the resident as well as the goals to be accomplished for each problem or need identified. Works together with care planning team to implement final plans. Encourages the resident and his/her “responsible parties” to participate in the development and review of care plans. Care plans must focus on assisting residents to reach their highest practicable level of well being. The MDS Care Plan Coordinator ensures that all nursing personnel are aware of the care plan for each resident and that care plans are used in providing daily nursing services. Reviews nurses’ notes and monitors the resident to ensure the care plans are being followed and if each residents’ needs are being met. Assesses, reviews and revises care plans as required. Plans, schedules and conducts weekly care plan meetings for all residents according to OBRA and state requirements. Completes the MDS with utmost accuracy and insures highest level of reimbursement for facility. Complies with current CMS Mega Rule guidelines. Point Click Care experience a plus. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. Summary: The Registered Nurse (RN) plays a critical role in executing InterCommunity mission to promote whole person health. Under the supervision and direction of the Clinical Practice Manager, the Registered Nurse is responsible for providing services which consists of an evaluation and assessment of medical and psychiatric status as an integral part of the interdisciplinary team. The Registered Nurse also provides direct patient care in the departments of primary care and psychiatry. Essential Duties & Responsibilities: Provides direct patient care, carrying out orders given by my medical provider, which may include obtaining medical histories, obtaining vital signs, performing office lab testing and phlebotomy/ venipuncture, EKGs, wound care, and which includes but is not limited to medication administration. Conducting nursing visits or educational groups as ordered by the medical provider Maintains accurate, detailed reports and records. Records and reconciles patients' medical information. Monitors, records, and reports symptoms or changes in patients' conditions, assess efficacy of prescribed medications. Ensure gaps in care for screenings, chronic care tests (such as HgbA1c for diabetes), BMI, vaccinations, are completed in timelines established by best practice. Facilitates effective crisis intervention. Perform safety assessments and triage for patients in crisis. Consults and coordinates with healthcare team members to assess, plan, implement, or evaluate patient care plans. Modifies patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes Maintains an active 10- patient panel for Complex Care Management (CCM). Performs patient triage of acute conditions in-person or over the phone. Refers patients to specialized health resources or community agencies furnishing assistance. Performs other duties as required. *All agency staff are required to attend all mandatory department/agency meetings and trainings Requirements Education &/Or Experience: Graduation from an accredited nursing program. Current State of Connecticut RN licensure required. One (1) year medical experience in a public health setting required. Current Connecticut RN License required. 2 years of community health, addiction medicine and/or psychiatric nursing required. Basic Life Support (BLS)/ CPR certification. EPIC experience preferred, but not required. Bilingual (Spanish Speaking) preferred, but not required.
