The RN coordinates care to achieve desired outcomes, participates in patient and family teaching and provides leadership by delegating and working cooperatively and collaboratively with ancillary nursing and other members of the healthcare team in maintaining standards for professional nursing practice and professional nursing performance in the clinical setting. The RN is also accountable to practice in accordance with the Michigan Public Health Code Act 368 of 1978, Article 15 (Health Occupations), Part 172 (Nursing). The RN must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area. $3,000 Sign-on Bonus!* Pay Range: $34.00 - $49.18/hour (float team incentive is an extra $1/hr!) Employment Type: Part Time, 48 hours every 2 weeks Schedule: 7pm to 7:30am, Friday and Saturday nights. Corewell Health is offering up to $10,000 in sign on bonuses for this opportunity! Weekend Option Program: Weekend RNs/LPNs will be required to work 48 out of 52 weekends per calendar year. Weekends are defined as Saturday and Sunday for day shift and Friday and Saturday night for night shift. RNs/LPNs can request up to 48 hours of PTO on their weekend shifts as well as 48 hours of exchanging a weekend shift for a weekday shift, for a total of 96 hours ‘off’ of the weekend requirement. This would be given in accordance with unit guidelines with allotted PTO as well as peak times, etc. RNs/LPNs may request 1 holiday off per calendar year that falls on the weekend. The RN/LPN would only work the holiday if it fell on the weekend, they will not be moved off a weekend to work on a holiday. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role. Provides clerical support which may include completing forms/requisitions, answering phones, sending/receiving medical records, obtaining lab/x-ray reports, hospital notes, and referral information. Cleans rooms between patient visits in accordance with infection prevention and control standards. Administers medications and immunizations under authorization and supervision of physicians and/or advance practice providers. Processes messages from patients and front office staff to physicians and advance practice providers. Processes messages and call backs to patients, medical offices and pharmacies. Processes medication refills according to protocols. Maintains all logs and required checks (i.e. refrigerator temperature, emergency medications, expired medications, oxygen, sterilization fluid charge etc.) Performs EKG’s and simple (CLIA waived testing) medical office laboratory and screening tests. Count and sign on approved form, all narcotics with approved personnel before accepting medication keys. Contact a supervisor immediately with discrepancies. Order all necessary medication and treatments from appropriate pharmacy or notify responsible party when supply is no less than 7 days.
