Review resident care plans and make recommendations based on assessment findings. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives. Educate and support staff regarding MDS processes and documentation standards. Monitor changes in regulations and ensure ongoing compliance. Qualifications: Current Registered Nurse (RN) license or Licensed Practical Nurse (LPN) license in the state of Illinois. Previous experience in MDS coordination in a skilled nursing or long-term care setting preferred. Knowledge of RAI process, MDS 3.0, and Medicare guidelines. Strong attention to detail and excellent organizational skills. Ability to work collaboratively with interdisciplinary teams. Proficiency with electronic health record (EHR) systems.
