Conduct medication reviews within nurse-led visits to ensure patient safety and adherence. Conduct onsite testing (including point-of-care labs, EKGs, and specimen collection) and use patient data to identify and close gaps in preventative screenings, ensuring compliance with state scope of practice. Review and act upon high-priority abnormal laboratory results in a timely manner to maintain patient safety and continuity of care. Partner with providers and the interdisciplinary team to manage complex care plans and coordinate weekly transitions for patients moving from the hospital back to the clinic. Utilize standardized protocols for medication management, prescription renewals and prior authorizations. Ensures clinical communications are managed, tasks are completed and patient visit encounters are closed in a timely manner. Delegates to medical assistants and phlebotomists as needed to support patient flow and efficiency. Performs other related duties as assigned. The Plaza Rehab and Nursing Center is currently seeking a Full -Time RN Unit Managers for day (7am-3pm), evening (3pm-11pm) night (11pm-7am) and Weekend shifts to join our dynamic organization. OVERVIEW: Unit Manager is primarily responsible for overseeing day to day patient care, supervising, directing and developing nurse staff, and reporting to the Director of Nursing to ensure quality patient care within a long-term care center, skilled nursing facility or assisted living community. RESPONSIBILITIES: · Identify patient service requirements by establishing personal rapport with potential and actual patients as well as nursing staff in a position to understand service requirements. · Assure quality of care by enforcing adherence to state board of nursing and state nurse practice act requirements · Maintain nursing staff by recruiting, selecting, orienting, scheduling and training nurses and auxiliary staff. · Maintain safe and clean working environment · Ensure operations of medical and administrative equipment verifying emergency equipment availability; completing preventive maintenance requirements. · Maintain a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods. Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. 4. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis.
