Job Description Utilizes the nursing process for assessment, planning, implementation, and evaluation to provide safe, therapeutic care. At this operational level, the nurse proactively embraces the Orthopaedic Society/Standards of Orthopaedic Nursing practice to utilize the nursing process for assessment, planning, implementation and evaluation to provide safe, therapeutic care. The nurse utilizes an interdisciplinary approach across the continuum of care with adherence to National Patient Safety Goals set forth by regulatory requirements. The nurse is recognized and utilized as a clinical resource and demonstrates investment in the overall forward movement of the unit/clinic and the organization. The nurse is responsible for bringing together the multidisciplinary efforts of the Attendings, Advanced Health Practitioners, Clinic Staff, Residents, Fellows, IP Nursing Staff, PT/OT, Case Management, Pharmacy, Practice Coordinators, Analysts, and all other ancillary and support departments to ensure a high quality, coordinated, and satisfying experience for the patient. The Nurse Navigator will be involved with the department and Health System quality metrics. Explains tests, procedures and disease processes to patients and their family as indicated; explains the major anesthetic risks to patients as requested and documents that the risks are explained to patients. Develops the care regimen of assigned patients; reviews the care regimen with the supervising physician and gains concurrence; implements the anesthesia plan; starts IV solution, administers selected anesthetic, choice of anesthetic consistent with patient safety, surgical need, possible post operative complications, and patients history and conditions. and evaluates and manages the patient, according to departmental policy. Participates with post anesthesia care unit personnel in a patient safety oriented interface when patients are transferred from the surgery suite. Monitors patients vital signs, such as blood pressure and observes any significant physical changes; informs anesthesiologist and/or surgeon of patients condition and takes any necessary remedial action such as infant resuscitation when necessary. Mixes solutions to required concentrations, such as pentothal, succinylcholine, vaso-active drugs etc. Keeps current accurate and complete record of procedures performed and patients conditions immediately prior to the administration of anesthesia according to established standards. Performs procedures done outside of OR, such as intubation, arterial line placement, etc., in accordance with policies and procedures, competencies, and physician order. This is NOT a remote position Title: Weekend Home Health RN (Field) Schedule: Ful Time (24 visits per week w/benefits) or Part Time (18 visits per week) - must be available to work up two (2) weekends per month * Non-traditional schedules are available, i.e., Thursday-Monday Locations: Area #8 covers the following cities: Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington AND Area #8 covers the areas of Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont Paid Training: Must be able to train for 5 days Mon-Friday 9am-5:30pm in San Ramon (onsite) Position Summary: The Home Health RN monitors a patient’s overall health, and in partnership with a primary care physician, develops an individualized plan of care for their patients. Each RN functions as a case manager, and works independently to deliver quality healthcare to their patients. As well as customized care and patient education about disease management, the Home Health RNs provide a variety of specialized services, such as Simple to Complex Wound Therapy, Post-Surgical Care Management, and Ostomy Care Management. We provide an entire internal operational support team for our clinicians to handle the tasks of pre-populating the medication reconciliation, tracking down DME orders, getting MD signatures, coding, lab follow-up, etc., truly allowing you to function independently, with the flexibility to make your own schedule and plan patient visits around other commitments if needed. The home care setting allows you quality time with your patient in the comfort of their own home. Requirements: Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting Must Possess an active CA Registered Nursing License AA, BSN, MSN Reliable Transportation Current CPR or BLS Card Benefits Include: Medical, Dental, Vision, 401K, Life Insurance, Holiday Pay and PTO Compensation is based on full time schedule
