CORE JOB SUMMARY The Registered Nurse 2 - Specialty delivers patient-family centered care in a culturally competent manner utilizing evidence-based standards of quality, safety, and service while ensuring population-specific patient care. The incumbent oversees nurses and allied health staff in their daily practice. CORE JOB FUNCTIONS 1. Assesses assigned patients and evaluates plans to include documentation of nursing care, and reports symptoms and changes in patients’ condition and vital signs. 2. Administers and maintains accurate records related to medications and treatments as per regulatory bodies, policies, procedures and physician orders. 10. Communicates plan of care in a timely manner to patient and family, as well as the appropriate team members, ensuring compliance with all regulatory guidelines (i.e., HIPAA). 11. Uses available resources to assist in discharge planning. 12. Plans, prioritizes, and adjusts assignments to accomplish goals and render superior patient care; seeks assistance when needed. 13. Safely operates medical equipment. Serves as a preceptor and assists new staff in the provision of care in order to help them acclimate to the healthcare environment, and a direct patient care role. 14. The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan. Responsibilities : Responsible for assisting patients with and per plan of care, the following: Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency) Safe transfer and ambulation techniques Exercise according to the plan of care Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency) Patient’s laundry Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.) Observation and teaching of home safety practices Recognition of an emergency situation and knowledge of emergency management procedures Must follow patients’ plan of care and complete all tasks assigned Communicates in a constant, clear, and effective way with the client, family/caregiver and office Observes and documents patient condition and reports any changes, to the assigned nurse supervisor Requirements : Meets all training requirements in accordance with state and federal laws when applicable Ability to read, follow written instructions and document care given Possess and maintains current CPR certification in accordance with state regulation PPD or chest x-ray if applicable Successful completion of Maxim screening examinations Participation and completion of Maxim’s competency program Must meet all federal, state, and local requirements The Lakes benefits : part-time position. Responsibilities: Utilize the nursing process to assess, plan, implement, and evaluate patient care. On each assignment, provides skilled nursing care/services in accordance with prescribed orders Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition. Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses. Description Job Summary Provides direct and indirect care duties under the guidance and direction of the registered nurse (RN). Performs a variety of invasive and non invasive technical skills and specialized patient care procedures as well as other duties to support 24 hours of hospital operations. Job Specific Duties Receives patient care assignment from the registered nurse (RN) in charge. Initiates delegated tasks based on demonstrated competence in performing nursing activities. Assists patients with activities of daily living to include bathing, personal hygiene, nutrition and comfort measures, changing bed linens, and diapers. Performs vital signs, heights, weights, head circumference & other measurements and records the results in the Electronic Health Record according to nursing and hospital policies & procedures.
