Three or more years of clinical experience in a setting committed to collaborative decision making and team development required. At time of hire, a record of completion and/or enrollment for all education, licenses, training and/or certifications is required. Continuous re-certification and/or maintenance of state licensures, certifications, educational and applications requirements is required during employment.
