MDS Coordinator - SNF Located in The Bronx, NY Salary: $130K to $145K range (based on experience) Responsibilities of the MDS Coordinator-Nursing Home: Completes assessments, Minimum Data Set (MDS) and care plans for all residents assigned Monitors completion of MDSs by other disciplines within timeframes prescribed by regulatory guidelines Advises supervisor of incomplete and/or untimely assessments by disciplines other than nursing. Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents. Initiates care plans and supporting activities that will result in best possible outcome for assigned residents. Generates and distributes monthly care plan calendar for the following month. Conducts care plan conferences for assigned residents. Job Responsibility Demonstrates expertise in Performance Improvement methodologies used throughout the organization. Maintains current knowledge of best practices, regulations, and advancements in healthcare quality, patient safety, and risk management. Participates in the preparation of Performance Improvement reports for care services presentation at committee meetings, and facilitates reporting to Hospital Quality Management for external benchmarking as needed. Assists in conducting focused studies to measure and assess the stability of patient care processes and the achievement of patient outcomes and organizational goals. Assists in coordinating investigations of state reportable incidents, complaints, sentinel events, and citations, ensuring thorough data collection for corrective actions. Participates in special projects related to quality and patient safety issues; assists in developing recommendations and coordinating the implementation of corrective action plans. Prepares monthly variance reports for leadership, physicians, and other hospital personnel as needed.
