The Patient Care Coordinator (PCC) is a Registered Nurse with a bachelor's degree in nursing who educate their patients about their health conditions and develop a care plan to address their specific discharge needs. They also facilitate communication between patients, families, caregivers, social service organizations and healthcare providers. The PCC works with healthcare team members to coordinate care and manage resources with a focus on returning home with necessary resources to ascertain quality, patient safety, and desired outcomes are achieved. The PCC partners with the unit teammates and leaders to facilitate effective flow and capacity evidenced by decrease LOS, and decrease readmission. Required Knowledge & Skills Education: Bachelor's Degree Experience: Two years of related experience. Requires working knowledge of specialized practices, equipment, and procedures. 4 Years RN related experience, preferred. The Float Pool RN performs nursing duties in accordance with hospital policy procedure, may function as a charge nurse or staff nurse, and supervises other nursing personnel who are responsible for patient care. Capable of making independent decisions in the majority of situations and is therefore able to direct patient care that is provided by others. Serves as a resource to other employees. Is responsible and accountable for prescribing and coordinating all nursing care provided on the unit utilizing the nursing process. Overview Shift’s Available: Days Employment Type: Full Time Hours: 12 hours Location: Southern Regional Medical Center - Riverdale, GA The Registered Nurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Responsibilities Essential Duties and Responsibilities (includes, but not limited to): Provides direct patient care within the scope of practice. The Registered Nurse also directs and guides patient/family teaching and activities of other nursing personnel while maintaining standards of care in Telemetry Unit. The Registered Nurse is directly responsible and accountable for the care given to his/her assigned patients. He / She communicates with the physician about changes in the patient's clinical condition including cardiac monitoring, results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition and/or response to treatment. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current and valid state RN License.2. Current BLS certificate upon hire and maintain current.3. Basic Arrythmia Interpretation within 30 days of hire. 4. Current ACLS certificate 30 days upon hire and maintain current.5. Previous Acute Care hospital experience preferred.6. Ventilator Management experience preferred.7. Medical-Surgical Nurse-Board Certified (RN-BC) or similar certification; preferred.8. Bachelor of Science in Nursing (BSN) preferred.
