The Mobile Hospital at Home Registered Nurse provides hospital-level acute care in a patient’s home within a defined radius around designated hospitals as part of the Advocate Hospital at Home team, which may include the following: Physicians, Advanced Practice Providers, Pharmacists, Case Managers, Social Workers, Physical Therapists, and other operational staff according to the Hospital at Home Care Model. We’re Looking For: Completion of an accredited or approved program in nursing Certification / Registration / License Registered Nurse license issued by the state in which the team member practices Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA). Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies.
