Overview Registered Nurse (RN) - Acute Care Paid Dialysis Training | $8,000 Sign-On Bonus Dialysis Clinic, Inc. Responsible for reports for monthly P&L meetings (e.g. P&L QI report, State QI report, Threshold Tracking Sheet & QA Meeting Minutes). Participate in surveys and inspections by government agencies; and, make reports and recommendations to the Administrator concerning operations of the department. Participates in department head and nursing department meetings. Assumes the duties of the Administrator in his/her absence. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Conduct clinical reviews for authorization requests using established criteria including Interqual, Mahalik, and health plan policy and procedures for inpatient, outpatient, Durable Medical Equipment (DME), Behavioral Health, and Private Duty Nursing. Participate in health plan interdisciplinary team conferences and collaborative case reviews to discuss complex cases and determine appropriate discharge plan or level of service. Consult with health plan medical director on an as needed basis to discuss medical necessity for requested service. Work closely with peers and other departments to determine discharge needs including necessary referrals to health plan care management for short or long term interventions. Maintain communication with health care providers regarding health plan determinations. Identify potential quality of care concerns and never events and refers to health plan quality management department. Minimum of 2 years of experience in a clinical and/or case management nursing required. Minimum of 1-year work-related experience in Utilization Management required. PA RN license strongly preferred. Work experience of 1 year discharge planning preferred. BSN preferred. Strong organizational, task prioritization and problem solving skills. Ability to construct grammatically correct reviews using standard medical terminology. Computer proficiency required. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Purpose: UPMC Health Plan is hiring a full-time Clinical Care Manager (RN) - Oncology Nurse Navigator to work as part of a multidisciplinary care team providing Oncology discharge planning and case management support. This role is Hybrid and will work Monday-Friday, standard daylight hours with a minimum of 2 days onsite at UPMC Hillman Cancer Center. The Clinical Care Manager is responsible for care coordination and health education with identified Health Plan members through face-to-face collaboration with members and their caregivers and providers. Identifies members' medical, behavioral, and social needs and barriers to care. Develops a comprehensive care plan that assists members to close gaps in preventive care, addresses barriers to care, and supports the member's self-management of chronic illness based on clinical standards of care. Collaborates and facilitates care with other medical management staff, other departments, providers, community resources and caregivers to provide additional support. Members are followed by face-to-face interactions in their community including the hospital, providers' offices, home, and other health care facilities. Responsibilities: Assist member with transition of care between health care facilities including sharing of clinical information and the plan of care. Document all activities in the Health Plan's care management tracking system following Health Successfully engage member to develop an individualized plan of care in collaboration with their primary care provider that promotes healthy lifestyles, closes gaps in care, and reduces unnecessary ER utilization and hospital readmissions. Coordinate and modify the care plan with member, caregivers, PCP, specialists, community resources, behavioral health contractor, and other health plan and system departments as appropriate. Review member's current medication profile; identify issues related to medication adherence, and address with the member and providers as necessary. Refer member for Comprehensive Medication Review as appropriate. Refer members to appropriate case management, health management, or lifestyle programs based on assessment data. Engage members in the Beating the Blues or other education or self management programs. Provide members with appropriate education materials or resources to enhance their knowledge and skills related to health or lifestyle management. Contact members with gaps in preventive health care services and assist them to schedule required screening or diagnostic tests with their providers. Assist member to schedule a follow up appointment after emergency room visits or hospitalizations. Plan standards and identify trends and opportunities for improvement based on information obtained from interaction with members and providers. Present or contribute to complex case reviews by the interdisciplinary team summarizing clinical and social history, healthcare resource utilization, case management interventions. Update the plan of care following review and communicate recommendations to the member and providers. Conduct comprehensive face to face assessments that include the medical, behavioral, pharmacy, and social needs of the member. Review UPMC Health Plan data and documentation in the member electronic health records as appropriate and identify gaps in care based on clinical standards of care. Minimum of 2 years of experience in a clinical setting and case management nursing required. Minimum of 2 years of Oncology experience highly preferred. Minimum 1 year of health insurance experience required. 1 year of experience in clinical, utilization management, home care, discharge planning, and/or case management preferred. BSN Preferred.
