As a RN, you will complete hospice admissions and patient visits as needed. Applicants must have great clinical and communication skills. The RN is responsible for answering calls from patients, families and customers. The RN will make nursing visits for symptom management, deaths, and admissions as needed. This RN must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice. This position will be after hours on-call working seven days/nights on and seven nights/days off. Qualifications Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license Admissions and On-call experience preferred Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership. Work closely with patients on a 1-1 level but get strong support from a patient care team. Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system. Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area

CTI operated collaboratively across the Northwestern Medicine health system to advance our mission to deliver the highest quality care for patients with diseases of the chest, promote clinical innovation and discovery, and improve lung health in the Greater Chicago Community. In this role you will work closely with our transplant pulmonologists and thoracic surgeons to support our growing inpatient Lung Transplant ICU Team. The APP will provide primary management to patients in the ICU as well as consultative coverage for transplant patients in the MICU or CTICU.
