MDS Coordinator

MDS Coordinator The primary role of the MDS Coordinator is to coordinate and guide the overall process and tracking of all Medicare/Medicaid case-mix documents, and care assessments in order to assure appropriate reimbursement for services. The position is also responsible for completing concurrent MDS to assure achievement of maximum allowable RUG categories, as well as integrating information from Nursing, Dietary, Social Services, and Rehabilitation departments for appropriate reimbursement. The Licensed Practical Nurse works under the direction of a registered nurse to provide nursing care, to assist the patient in learning self-care techniques and to prepare clinical and progress notes. Reports off to RN case manager and/or clinical manager. 3. Reports all changes in condition to MD. 4. Provides patient/caregiver education as per care plan. 5. Completes all documentation within 24 hours. Must include changes in condition, follow-up with provider or other disciplines with supporting documentation. 6. Attends all staff meetings, inservices, and all other meetings as required. 7. Compliance with all mandatory requirements -inclusive of Healthstreams. 8. Follows policies and procedures. Qualifications EDUCATION Graduated from an approved Practical Nurses Education Program EXPERIENCE Minimum one year experience in the acute care setting. Home care experience preferred, but not required. LICENSURE License Practical Nurse with valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire. SPECIAL SKILLS Excellent verbal and written skills. Excellent clinical knowledge. Strong computer skills. PHYSICAL DEMAND Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Staff Nurse - Sedation - 36hr

SIGN ON BONUS OFFERED TO ELIGIBLE APPLICANTS Under general supervision, assumes 24-hour accountability to plan and provide direct professional nursing care for specified patients throughout the entire episode of service delivery as defined by population and unit. Provides nursing services to patients and families in accordance with the standards of practice of a professional nurse, CCMC policies & procedures, TJC, and State & federal regulations. Reports to Nurse Manager. Clinical Practice- On a daily basis, demonstrates clinical expertise and uses the nursing process to provide safe, appropriate patient care to a defined patient population according to CCMC and unit policies and standards. Population Specific- Demonstrates knowledge of the differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families. Unit Leadership- Delegates duties and tasks associated with patient care to clinical staff according to hospital policy and their scope of practice. As assigned, contributes to the leadership of the unit by functioning as the Resource Nurse and/or by participating in the professional development of others through preceptor/mentor activities.
