Description The Home Health Nurse Manager is responsible for the management and oversight of clinical staff for the licensed home care agency and nursing staff supporting home infusion services under the guidance of the Director of Patient Services, S/he ensures that personnel issues are managed in compliance with Department of Health (DOH) regulations and contractual agreements. Responsibilities Patient Experience 1. Supervises all clinical staff of Mount Sinai Home Health LLC home care agency and nursing staff supporting home infusion services. 2. Responsibilities: A Registered Nurse who, under the direction of the nursing leadership is responsible for planning, directing, implementing, coordinating, and evaluating nursing care for a specific unit during her/his shift. Receives report from previous shift, making sure that he/she has received all pertinent information on each resident. Continuously evaluates behavior, symptoms, reactions and progress of residents and discusses findings with Nurse Manager/Charge Nurse. Administers and documents all medications according to established policies and procedures, physician's orders and Department of Health regulations. Performs all treatments according to physician's specific order and established policies and procedures. Discusses the need to discontinue or change a medication of treatment with the Nurse Manager/ Charge Nurse. Orders prescribed medications, notifies pharmacy of discontinued medications: and ensures accuracy of medications and dosages when received from pharmacy. Overview Carries out patient care responsibilities with emphasis on decision-making related to the steps in the nursing process to include assessment, diagnosis, and identification of expected outcomes, planning, implementation and evaluation. As a member of the interdisciplinary healthcare team, formulates the initial plan of care with the provider (physician or nurse practitioner) and collaborates with them to implement a comprehensive patient-centered plan of care to achieve expected outcomes; utilizes principles of assignment and delegation consistent with the NY State Nurse Practice Act. Responsibilities Provides professional nursing care to patients within unit. Diagnose and treat response to actual or potential health problems by means of nursing techniques and methods such as case finding, health teaching, health counseling and the provision of care supportive to and restorative of health and well-being. Maintain safe levels of care while adhering to a medical regimen. Collaborates with the physician and other health care providers to assure that a goal oriented care plan is consistent with medical regimen and hospital procedures. Performs other related duties as instructed. Qualifications BSN with three (3) years’ experience preferred. Current NYS license to practice as a Registered Professional Nurse. BLS Required. Bilingual Preferred: English/Spanish.
