Job Summary In the ICU of a hospital, patients are often in critical condition and need constant monitoring and care. ICU nurses review data from special equipment to track a patient’s heart rate, blood pressure, breathing and other vitals. They also carry out more traditional nursing duties such as administering medications and communicating with patients regarding their condition. Essential Functions Performs a thorough and timely assessment of patient’s condition including physical, psychological, social and rehabilitative aspects. Identifies problems/nursing needs including discharge needs. Intervenes appropriately to address identified problems and needs. Evaluates effectiveness of care and revises plan if necessary. Transcribes, verifies and carries out physician orders in an accurate and timely manner Manages time efficiently/sets priorities appropriately. Initiates and effectively maintains IV therapy. Coordinates interdepartmental activities (i.e. lab, radiology, etc).To ensure timely, efficient care. Coordinates interdepartmental transfer smoothly and efficiently. The Advanced Practice Nurse (APN) provides medical assessment, diagnosis, and treatment of individuals experiencing substance-related withdrawal in a Withdrawal Management (Detox) setting. The APN functions under a collaborative agreement with a physician and is responsible for ensuring safe, evidence-based withdrawal management, medication management, and coordination of care in compliance with New Jersey Department of Health (DOH) and DMHAS regulations. Essential Functions Clinical Care & Withdrawal Management Perform comprehensive medical assessments upon admission and throughout withdrawal management. Diagnose and treat substance withdrawal syndromes, including but not limited to alcohol, opioids, benzodiazepines, and stimulants. Develop, implement, and adjust individualized withdrawal management plans, including medication protocols. Prescribe, administer, and monitor withdrawal-related medications (e.g., benzodiazepines, clonidine, buprenorphine, naltrexone, supportive medications) in accordance with scope of practice and collaborative agreement. Utilize standardized withdrawal tools (e.g., CIWA-Ar, COWS) and document clinical decision-making. Monitor for and respond to medical or psychiatric emergencies, including escalation to physician or transfer to higher level of care when indicated. Medication Management Conduct medication reconciliation on admission and discharge. Ensure safe prescribing practices, medication monitoring, and adherence to program protocols. Educate patients on medications, side effects, and treatment expectations. Collaboration & Care Coordination Collaborate with the Medical Director, nursing staff, counselors, and interdisciplinary team to ensure continuity of care. Participate in treatment team meetings, case reviews, and discharge planning. Coordinate referrals to ongoing SUD treatment, MAT programs, primary care, or higher levels of care. Participates in unit and hospital based quality, patient safety and patient experience process improvement initiatives Utilizes the principles of Lean methodology for process improvement through the use of the Daily Management Boards. Reviews unit progress, changes, and compliance with quality and safety metrics with clinical nurses and other members of the healthcare team. Monitors institutional information technology infrastructure and corrects gaps in clinical nursing documentation. Leads staff participation in quality and safety initiatives and mandated compliance measures. Demonstrates accountability for all unit quality and safety practices and compliance, and ensures required evaluation of staff competencies is maintained. Interprets and implements the associated collective bargaining agreements and System and Hospital policies and procedures. Performs disciplinary actions as necessary with collaboration with Nursing and Labor Relations. Acts as a management witness in disciplinary activities. Insures all staff have met their orientation goals/weekly progress reports and annual competency requirements and participates in the Competency Planning Tool for unit/service. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. Maintains records of minutes for staff team meetings. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. Facilitates organizational change initiatives by using effective strategies. Leads a shared decision making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. Responds to protest of assignment in a timely manner as detailed in the collective bargaining agreement and discusses the resolution in unit staff meetings.
