Practices proper infection control techniques. Follows facility infection control policies and procedures. 2. Maintains meticulous personal hygiene and grooming of the resident. Give showers, bathes, and shampoos residents as assigned. Besides bathing and incontinent care daily AM and PM care includes oral hygiene, as well as denture care, shaves for men daily and women as needed, inspecting fingernails to ensure they are clean and well-trimmed, washing inspecting skin for red or open areas, bruises, skin tears, reports to Charge Nurse. 3. Toileting and/or incontinent care is provided according to the facilities policies and procedures. 4. Encourage resident to participate in active ROM when possible and perform passive ROM according to the facility policies and procedures. 5. Residents are assisted before, during, and after meals by placing residents in an upright position, ensuring teeth are in, glasses are on, setting up food in front of resident checking that resident has correct meal, at correct consistency and using Thickener if required. Give resident as much assistance as required (cut meat, open containers) encourage the resident to eat and/or feeds resident as indicated on the assignment sheet. Record resident's intake and repost to nurse if resident's intake is low meal is refused, etc. 3. CLINICAL DUTIES CONTINUED: 6. Transfers residents according to method indicated on assignment sheet. Follows policies and procedure on body mechanics and use of resident lifts. Notify nurse if in doubt of resident's capabilities or questions about transfer method to be used. 7. Ambulates residents as directed by assignment sheet, Charge Nurse or Physical Therapy. 8. Obtains the resident's vital signs and weight according to shift routine or as directed by Charge Nurse. 9. Answers calla lights promptly for any resident and addresses the resident's needs 10. Responds immediately to bed/chair alarms, Code Alert or door alarms. 11. Collects specimens of urine, stool, and sputum as directed by the Charge Nurse. 12. Passes out fresh water and ice, nourishments and snacks to residents as assigned, being sure to follow the resident's diet. 4. CLINICAL DUTIES CONTINUED: 13. Ensures safety of all residents. 14. Gathers and transports residents to recreational activities. 5. Accompanies residents to appointments at MD offices as directed. 6. Transports the resident to and from appointments with the facility. 7. Accepts changes in assignment as designated. 6. COMMUNICATION: 1. Communicates in a pleasant, polite, and helpful manner to residents, visitors, coworkers. 2. Works as a team member with co-workers. 3. Attends in-service demand mandatory by the facility. 3. Attends at least 12 hours of in-service training in a calendar year (completed by December) as required by OBRA regulations. 10. Scope Of Position The professional nurse practices, delegates, and collaborates with other team members to facilitate the coordination and delivery of patient care for the assigned patient population. The patient population is identified as neonate, infant, and child, adolescent, adult and older adult. The ODS and PACU Registered Nurse focuses on the pre and post operative needs of the patient. Reports to Manager, ODS/PACU. Responsibilities Nursing Process - Provides quality care to all patients by utilizing the nursing process. The Registered Nurse is responsible to assess, plan, develop and implement a plan of care with appropriate interventions for the pre-operative and post anesthesia/post-operative patients. Leadership – Demonstrates leadership to ensure all patient/family needs are met, regardless of race, religion, sex, age, disability, social or economic status while considering age specific factors. Applies the principles of case management to patient care, and have the autonomy, capability and motivation to be identified as one of the principal care givers for the patient, providing perianesthesia patient teaching, referring services and communicating pertinent information across the continuum of patient care. Manages and directs patient care in collaboration with other health care professional including surgeons, anesthesia providers, and assistive personnel. The Registered Nurse contributes to the efficient operation of the pre-assessment area, pre-operative area, and post-anesthesia care units by delegating responsibilities to the appropriate team members based on their scope of practice, knowledge and capabilities to ensure optimal utilization of personnel. Directs accesses and provides patient care to outpatients as well as inpatients. Responsible for assisting physicians/providers with patient care, rooming patients, and performing clerical, environmental, and organizational tasks. Identifies self to patient/family and explains to patient/family the roles of each member of the care delivery team. 1.2. Accompanies patients to the exam/procedure room. 1.3. Prepares patients for visit. Typically includes vital signs, height, weight, blood pressure, EKG, specimen collection (where applicable), phlebotomy (where applicable), INR (where applicable), pulse oximetry, spirometry, etc. Documents medical and surgical history, family history, social history, problem list, and medications in EPIC. 1.4. Performs screening per physician/provider guidelines. 1.5. Assists physicians/providers with various procedures. 1.6. Obtain pre-authorizations when necessary. 1.7. Convey instructions to patients/family members/caregivers. 1.8. Provides appropriate resource materials and referrals to patient/family as instructed by physician/provider. 1.9. Maintains patient records effectively. 1.10. Efficiently manages and routinely checks the EPIC in-basket. 1.11. Answers and returns calls promptly and provides pertinent information. 2. Performs administrative responsibilities as assigned. 2.1. Sends/receives patient medical records and abstracts new patient records into EPIC. 2.2. Obtains lab/radiology reports, hospital notes, and referral information as requested. 2.3. Completes forms/requisitions as needed. 2.4. Cross-trained on front desk duties, including but not limited to greeting patients/visitors, answering calls, scheduling appointments, and maintaining/scanning patient medical records. 3. Performs environmental responsibilities as assigned. 3.1. Sets up instruments and equipment according to department protocol. 3.2. Applies principles of infection control. 3.3. Cleans exam/procedures rooms, instruments, and equipment between patient visits to maintain infection control following established department policy. 3.4. Cleans the sterilizer (when applicable) according to the scheduled maintenance program and maintains appropriate documentation. 3.5. Orders, sorts, and stores supplies following established department policy. 4. Performs other related organizational duties as assigned. 4.1. Maintains patient autonomy, confidentiality, and dignity. 4.2. Ensures providers submit timely and accurate billing. 4.3. Maintains knowledge of practice policy, HIPAA, current coding, OSHA regulations and CLIA regulations (where applicable). 4.4. Provides prompt attention to urgent and non-urgent patient issues as evidenced by observation and feedback from staff. 4.5. Quickly shares problems related to patients and/or staff with the supervisor. 4.6. Work with the Care Coordinator for Patient Centered Medical Home initiatives. 4.7. Attend and actively participate in staff meetings. 4.8. Collaborate in the development and delivery of programs and activities for individuals, families and population groups that promote health and prevent disease. 4.9. Collaborates in the development of and contributes to individual team and department quality improvement and evaluation activities. 4.10. Advocate on behalf of vulnerable individuals and populations. 4.11. Contribute to a work environment that fosters ongoing educational experiences for self, colleagues, healthcare professionals and members of the community. 4.12. Collaborate in developing a work environment where continuous quality improvements in practice are pursued. 4.13. May assist and/or perform chaperone duties for patient medical procedures as needed. 6. May perform other duties as required. Qualifications EDUCATION High School diploma or equivalent required. Must meet one of the following requirements: graduation from an accredited medical assistant (MA) certificate program or Phlebotomy program, certification as a certified nursing assistant (CNA), emergency medical technician (EMT), or paramedic. Employees who join YNHHS through a merger or acquisition may satisfy educational requirements by demonstrating the appropriate combination of education, relevant experience, and current competencies. EXPERIENCE Minimum of one (1) year of experience as a medical assistant (MA), certified nursing assistant (CNA), emergency medical technician (EMT), ambulatory care associate (ACA), patient care associate/technician (PCA/PCT), paramedic, or phlebotomist required. New graduates from an accredited medical assistant certificate program, with less than one (1) year of experience, will be considered. LICENSURE Certification/registration (CCMA/CMA/AAMA/RMA) preferred. Certification/registration may be accepted in lieu of graduation from an accredited MA school. Certification in Basic Life Support (BLS) preferred. SPECIAL SKILLS Ability to follow directions effectively and consistently.
