HOW YOU'LL WORK: You'll provide skilled nursing care in a patient's home as directed by the attending physician. You'll work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Observe and monitor patient conditions and perform OASIS assessments (If you don’t already know this assessment, we will teach you!) to develop an individualized care plan and adjust as needs change. Minimize Patient Risk: Administers medication as prescribed by the physician and helps decrease re-hospitalizations by prioritizing visits for high-risk patients. Continuity of Care: RN will manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Educate the patient and family on the disease processes using teach back methods to ensure patient and family understanding. Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborates with the interdisciplinary team and physicians to ensure optimal care is provided. Rules and Regulations: understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors. Position: REGISTERED NURSE Department: MIXED ACUITY Shift: FULL TIME / 7P - 7:30A / 72 HOURS PER PAY Facility: Miami Valley Hospital North (MVHN) Sign on Bonus * The Clinical Nurse is a registered nurse (RN) who partners with and advocates for patients/families/significant others to provide physical, emotional and spiritual support which impacts patient outcomes in a safe environment. The RN functions in a versatile role that demonstrates professionalism and promotes excellence and autonomy in the practice of Nursing. The RN utilizes the nursing process in conjunction with evidence-based principles to care for a diverse caseload of patients within the department scope of service. The RN serves as guide, healer, collaborator, teacher, leader and sentry. The RN collaborates with and guides care delivered by the inter-professional team. The RN is accountable for compliance with the Ohio Nurse Practice Act, Nursing Code of Ethics and applicable regulatory standards. Education: Graduation from accredited school of nursing; BSN preferred. Licensure/Certification/Registration  Valid Ohio RN license  Current CPR certification required at the time of hire. Experience: No prior job-related work experience Knowledge/Skills Must be able to multitask with constant interruptions while maintaining a pleasant demeanor Basic computer knowledge Strong organization skills Effective interpersonal and communication skills
