Description The Home Health Nurse Manager is responsible for the management and oversight of clinical staff for the licensed home care agency and nursing staff supporting home infusion services under the guidance of the Director of Patient Services, S/he ensures that personnel issues are managed in compliance with Department of Health (DOH) regulations and contractual agreements. S/he works in collaboration with clinical leadership, administrative leadership, providers of care and referrers to ensure that the service maintains the highest standards of patient care and is responsive to the needs of the patient and family and the organization. Responsibilities Patient Experience 1. Supervises all clinical staff of Mount Sinai Home Health LLC home care agency and nursing staff supporting home infusion services. 2. Oversees the agency orientation for all newly hired clinical staff including introduction to agency policy and procedures, clinical skills training and assessment, documentation and EMR requirements and competency evaluations. 3. Ensures clinical staff adherence with written policies and procedures 4. Performs field supervision visits with all clinical staff on an annual basis at a minimum and remediation supervision visits as needed. 5. Monitors data regarding patient census staff productivity and collaborates with Director of Patient services to ensure financial stability. 6. Participates in the recruitment and selection of personnel. 7. Oversees the maintenance of patient clinical records and clinical staff personnel files. 8. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of patient experience. 9. Synthesizes quantitative and qualitative feedback regarding the patient experience and staff engagement to Nursing Management, staff and the Interdisciplinary Patient Care Team that leads to the development and implementation of an action plan. 10. Overview Registered Nurse (RN) – Patient Admission Unit Position Summary The Registered Nurse is responsible for providing safe, compassionate, and evidence-based nursing care to patients admitted to the inpatient unit. The RN completes comprehensive admission assessments, develops individualized nursing care plans, coordinates patient care, and ensures accurate documentation and communication with the healthcare team. Job Details Key Responsibilities Receive and assess patients upon admission to the unit. Obtain and document comprehensive nursing histories, vital signs, medication history, allergies, and other relevant clinical information. Perform a thorough head-to-toe assessment and identify actual or potential patient care needs. Review admission orders and initiate appropriate nursing interventions. Reconcile medications and communicate discrepancies or concerns to the appropriate provider. Develop and implement individualized nursing care plans based on patient needs. Educate patients and families regarding the plan of care, medications, treatments, safety precautions, and unit procedures. Monitor patients for changes in condition and promptly report significant findings to the healthcare provider. Administer medications and treatments safely according to physician/provider orders and hospital policies. Maintain accurate and timely electronic medical record documentation. Coordinate care with physicians, nursing staff, case management, pharmacy, rehabilitation, and other members of the interdisciplinary team. Maintain patient safety, including fall prevention, infection control, and appropriate use of safety precautions. Participate in handoff communication and ensure continuity of care. Assist with discharge planning and provide appropriate discharge education. Follow hospital policies, nursing standards, regulatory requirements, and evidence-based practices. Participate in quality improvement and patient safety initiatives. Graduate of an accredited nursing program. Current BLS certification. Previous acute-care or inpatient nursing experience preferred. Strong assessment, communication, organization, and critical-thinking skills. Ability to work effectively in a fast-paced multidisciplinary environment. Salary Range USD $54.48 - USD $74.72 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
