Job Description Medical Center: Outpatient Surgery Center - Full-Time, Day Shift Job Summary: The Clinical Nurse C demonstrates competency in prescreening patients prior to procedures requiring general anesthesia or monitored anesthesia care. Facilitates coordination of care and communication among the healthcare team to achieve optimal surgical outcome. Works closely with the surgeon offices to retrieve pre-operative tests (EKGs, Labs, Chest X-ray) results, history and physical, current medications, allergies, and other pertinent information related to diagnosis, type of surgery and comorbidities. Identifies potential issues that could possibly result in a negative surgical outcome, delay or cancellation of surgery. Provides education about surgery and recovery process to the patient, family or caregiver prior to surgery as appropriate. Performs the nursing process (assessment, care planning, intervention, evaluation) independently and in accordance with LLUMC established Professional Practice Model (Duffy's Quality Caring Theory) to achieve proficient individualized physical, psychosocial and spiritual wellness. Functions within the expectations of an accomplished professional nurse and meets nursing excellence standards to maintain Magnet Recognition as outlined by the American Nurses Credentialing Center. Inspires and adopts the transformation of evidence into point-of-care practice to optimize patient outcomes. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed. Education and Experience: Bachelor's Degree in Nursing required for external hires. Exception(s) may be granted as follows: 1) if currently enrolled in a nursing bridge program (AS degree to DNP degree), proof of 25% program completion required. DNP degree to be completed two and a half years post hire date. On the day-to-day, you will: Conduct home health nursing assessments to determine the nursing equipment needs in the home, including participant preferences and goals Actively participate in IDT meetings to develop participant care plans Deliver and document home health nursing interventions as agreed upon in participant care plans, including but not limited to maintaining a healthy and safe environment, promptly responding to physician orders, and accurately administering medications and performing ordered tests and treatments Instruct participants, their families, and caregivers regarding the disease process, self-care techniques, and prevention strategies related to their medical conditions Track and monitor home health scheduling in collaboration with the Home Health Manager or scheduling point of contact Collaborate with the Physician and other IDT members to assist in smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.) Job requirements include: Graduate of an accredited school of nursing Unencumbered RN License required Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population; home health agency experience preferred Reliable means of transportation Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred We are seeking Home Health RNs that ideally have experience working with frail and elderly populations. Illumination Health + Home is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IH+H currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire. Job Description The Medical Coordinator is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $30.00 - $35.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. The schedule for this position is Thursday - Monday from 7:00am to 3:30pm. Workdays may vary. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge.
