About the Department: The environment on our unit is fast-paced, multidisciplinary, educational, high-acuity, supportive, team-oriented, and compassionate! We care for Adult patients aged 18 to 118 who are being treated for post-op recovery and intensive care for coronary artery bypass grafting, aortic surgeries, valve surgeries, pulmonary endarterectomies, heart/lung/dual organ transplant, and care of temporary mechanical circulator support, including the majority of our center's ECMO population. Your Schedule: Full-Time Night Shift - 6:45 pm - 7:15 am (three 12-hour shifts/week) Shifts rotate throughout the week Note: Set schedules are not available. Schedules are determined by unit clinical staffing needs. All Employees (Night shift & PRN included) are required to attend hospital orientation during regular daytime hours during the first week of employment. What You Need to Qualify: Required : High School Diploma or GED (or equivalent experience) AND one of the following: Current enrollment in a professional nursing RN program with successful completion of the first semester ( including nursing fundamentals clinical ), or Graduation from a Nursing Assistant or Aide Training Program within the past year (certificate of completion required ), or Minimum of one year of recent experience ( within the past two years ) as a care partner, nursing assistant, or patient care technician. Medication Reminders Medication support is limited to the responsibilities authorized in the client’s approved non-medical care plan. Caregivers may: Provide scheduled reminders. Assist with an approved prefilled medication planner. Place approved medications in a cup when directed. Remind the client to self-administer their medication. Document reminders and client responses. Report refusals, missed doses or other concerns. Caregivers must not independently administer medication, change medication instructions or provide medical advice outside their authorized scope. Transportation When required and authorized, caregivers may: Transport clients to appointments. Assist with approved errands. Transport clients to social activities. Help clients enter and exit the vehicle. Secure mobility equipment appropriately. Follow all client-specific transportation instructions. Maintain a clean, safe and insured vehicle. Caregivers may also be considered for additional assignments when their experience, availability and location match current needs. Once a caregiver accepts an assignment, dependable attendance and completion of the entire scheduled shift are required. Employment Qualifications Applicants must: Be at least 18 years old. Caregivers must immediately report any change affecting their driver’s license, insurance, vehicle availability or ability to transport clients safely. Attendance and Communication Expectations Dependability is essential in home care. Caregivers must: Arrive on time for every accepted assignment. Confirm assignments according to company procedures. Maintain reliable telephone communication. Respond promptly to scheduling messages. Notify the office immediately if an emergency affects attendance. Follow all call-off procedures. Clock in and out from the approved client location. Remain with the client until the scheduled end of the shift. Remain until properly relieved when continuous care is required. Never leave a client unattended when supervision is required. Never leave early without office authorization. Notify the office if a client or family asks them to arrive late, leave early or change the scheduled hours. Complete all required documentation before the end of the shift. Failure to report for an accepted shift, leaving a client without authorization or failing to follow safety procedures may result in removal from the assignment or additional corrective action. Documentation Requirements Caregivers must document services accurately and in real time. Required documentation may include: Completed care-plan tasks Personal care provided Meals and hydration Medication reminders Toileting and incontinence care Repositioning Blood glucose-related support Client behavior and mood Changes in condition Skin observations Incidents Office notifications Transportation Incomplete tasks and the reason they were not completed Caregivers must never mark a task complete when it was not performed. Background and Screening Requirements Applicants must: Complete a comprehensive background check covering up to 15 years, where legally permitted. Complete required abuse, neglect and sex-offender registry checks. Complete a pre-employment drug screening when applicable. Participate in random drug screening when required by company policy. Provide complete and accurate application information. Report criminal charges or legal changes that may affect their ability to provide safe care. Have no disqualifying history that would prevent safe employment, subject to applicable laws and individual review.
