Per Diem Primary Function: The RN Care Manager (RN CM) performs utilization management, case management and discharge planning activities for acute care, and/or extended care patients including ambulatory and emergency department patients. The RN Care Manager works with the patient health care teams to assess patients transition/discharge planning needs, actively prevent unnecessary admissions/readmissions, and facilitate timely transitions/discharges. Job Qualifications and Specifications: If hired after January 01, 2016, a Bachelors of Science in Nursing from an accredited nursing school, college, or university is required. Master degree preferred. Must have and maintain license as a Registered Nurse in Rhode Island. Minimum of 3 to 5 years related clinical experience as a Registered Nurse. Case Management experience or knowledge required. Knowledge is defined as applicant has 1) a Nursing Care Management Certificate of Graduate Studies or 2) demonstrates current knowledge of Care Management through applicant pre-hire assessment. As of January 01, 2016, Certification in Case Management (CCM or ACM) must be earned within 24 months of employment, prorated for part-time staff. Ability to effectively communicate with and gain cooperation of resident and attending physicians with regards to documentation needs and to effectively influence all members of the interdisciplinary care team during difficult cases to achieve appropriate resolutions. Must have effective written and oral communication. The ability to interpret medical data as documented. Utilization management, quality management and basic knowledge of third party payers is preferred. Knowledge and training in multiple computer systems required to perform position responsibilities (i.e. EMR, Microsoft Office, etc.). Participation in weekend, and holiday coverage rotation. Flexibility to cover as needed. Job Summary The Providence Center ACT Engagement and Diversion Team is looking for a motivated, team-oriented Registered Nurse, to help assess and facilitate the tasks and duties listed below for adults with co-occurring conditions, medical complexities, and who are also dealing with unstable living arrangements. Experience with the chronically homeless and addictions populations a plus, and within our new "CCBHC" (Certified Community Behavioral Health Clinic") capacity. Duties & Responsibilities Triage and assess assigned consumers on the team. Assess and evaluate client health care data collected through interviewing, observation, record review and team input. Develop an individualized consumer plan of care. Perform integrated care in accordance with appropriate medical protocol. Identify expected outcomes of care, individualized to the client, with the ultimate goal of improving client’s overall health. Implement the consumer plan of care. Evaluate the consumer's response to the plan of care and update the plan based on consumer's response. Provide support to consumers regarding daily living issues, conducting community and home visits as necessary. Instruct consumers and families concerning the disease process, plan of treatment and rehabilitation course, using available teaching materials when appropriate. Educate consumers on the various medications on the expected effects and side effects. Question any aspect of the consumer's treatment regimen which endangers the level of wellness. Administer medications as per prescriber orders (Injections and PO). Implement appropriately prescribed medical regimen while following the 6 rights of medication administration. Monitor lab results and complete related documentation Ensure that externally prescribed medications (from outside providers i.e. PCP) are accurately documented in EMR. Liaise with treatment team and pharmacy regarding medication changes to ensure clients receive accurate and most current prescribed medications. Coordinate activities regarding consumer hospitalization. Follow up with clients post-hospital discharge to provide support and education regarding recommendations resulting from inpatient stay. Complete medication reconciliations, review hospital discharge paperwork, and communicate any changes or significant findings to treatment team, including MD. Collaborate with the staff members in providing care to consumers. Perform recordkeeping in accordance with Agency requirements. Complete initial and annual assessment forms. Maintain cooperative relationships with consumers, staff and the public. Communicate with various local agencies, medical facilities, and other departments within TPC, including pharmacy. Attend trainings, case presentations and conferences as needed. Perform duties in accordance with departmental goals, objectives and standards. Serve on appropriate committees. Partake in training new hires and student Nurses. May be asked to share on-call with other nursing and program staff. Requirements Associate’s degree in Nursing required; Bachelor's degree from accredited four-year College or University preferred. Bilingual preferred. Rhode Island Registered Nurse (RN) license or Compact License required. Experience in Human Services or Outpatient setting preferred. Experience working with psychiatric and substance use disorder patients also a plus. Position may require driving.
