Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Responsibilities Fulfills patient care responsibilities as assigned which may include: performing venipuncture and/or EKGs, checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring and dressing, as well as collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. Fulfills clerical responsibilities as assigned which may include: sending and receiving patient medical records; obtaining lab/x-ray reports, hospital notes, referral information, etc. Completing forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. These responsibilities may be carried out using the electronic medical record (EMR) system or manually as necessary. Enters orders into the electronic medical record (EMR) system or prepares prescription refill requests on behalf of the physician for the review and approval by the ordering physician. Inventories, orders or re-stocks medical supplies as applicable. Provides set-up of examination room and instruments in accordance with proper sterilization techniques established by hospital and infection control policies. Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, crash cart, point of care testing or others as needed. Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy. Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations. Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances. Assists in the maintenance of medical charts (filing, Op Reports, test results, home care forms). May schedule surgeries/procedures or work in conjunction with Surgical Coordinator verifying times with patients; preparing charts, pre-admissions and consent forms as necessary. May be required to perform proficient phlebotomy duties. May perform appointment scheduling and registration for patients as well as updating patient demographic and insurance information. May maintain, adjust and confirm patient office schedule. May check and verify and update insurance information utilizing internal or web sources. May perform and verify pre-certification for procedures as necessary. Performs other duties as assigned. Qualifications High School Diploma/GED with completion of Training Certificate from an approved Medical Assistant certificate course. Certified Medical Assistant preferred. Three to nine months of healthcare experience preferred. Description The Assistant Nurse Manager for Heart (Cath Lab Stepdown) is a Registered Professional Nurse with broad knowledge and skills who assumes the role of a clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager. Responsibilities Patient Care 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Description Job Title - Nurse Practitioner-Transcatheter Aortic Valve Replacement Program - Mount Sinai Hospital-Full Time - Day A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patients condition that falls within their specialty area of practice. NOTE: Medical orders, progress notes and treatments plans are not required to be countersigned by a physician. PRINCIPLES DUTIES AND RESPONSIBILITIES CLINICAL PRACTICE Assesses the health status of patients/families by means of comprehensive health history and physical examination. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. Documents findings and plan of care in the patient's medical record. Consults with physician according to established standards, policies, procedures, and protocols. Description Provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Mentors and assists less experienced staff members. Responsibilities 1. Description As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Description The Nurse Manager is responsible for the management of nursing practice and operations on a designated unit/service. This includes, but is not limited to, the management of: clinical nursing practice, staff recruitment, retention, labor relations, supply chain and budget, staffing, employee performance evaluation, quality improvement, staff education, medical records, internal and external regulatory and survey requirements. The Nurse Manager collaborates with multiple professionals to support and coordinate the provision / management of patient care. He or she works closely with the Nursing Clinical Director, unit Dyad partner and staff in insuring that the Service/Unit maintains the highest standards of patient care and is responsive to the needs of the patient & family as well as the organization. Responsibilities PATIENT CARE 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4. Validates staff members round hourly on all patients by direct observation and patient feedback. 5. Assesses staff competency in PHR and provides consistent meaningful feedback with more focused feedback as needed. 6. Conducts rounding of all patients on a daily basis (Monday-Friday.) 7. Communicates quantitative and qualitative information regarding the patient experience and staff engagement to the staff and the Interdisciplinary Patient Care Team that leads to the development and implementation of an action plan. 8. Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. 2. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. 3. Works collaboratively with physician dyads and all members of the healthcare team to promote patient care and regulatory standards. 4. Maintains a nursing unit in compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. 5. Oversees 24 hour patient care delivery, throughput, staffing and processes to ensure efficient and effective operations. 6. Prepares, implements and monitors unit budgets, including capital equipment allocation, budget variances, management of OT and vacancies according to MSHS financial goals. 7. Maintains control of established positions and manages the recruitment and retention of qualified staff. 8. Maintains patient/employee confidentiality in the management of information. 9. Evaluates nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. 10. Monitors availability and maintenance of nursing equipment and supplies. 11. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. 12. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. 13. Facilitates organizational change initiatives by using effective strategies. 14. Leads a shared decision making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. 15. Interprets and implements collective bargaining agreements affecting staff. 16. Responds to protest of assignment in a timely manner as detailed in the collective bargaining agreement and discusses the resolution in unit staff meetings. PROFESSIONAL DEVELOPMENT 1. Overview Provides mental health assessment of and treatment to patients with psychiatric symptoms, cognitive deficits, and/or behavioral problems. Provides support and education to VNS Health staff and caregivers to enable them to better care for patients with psychiatric, cognitive and/or behavioral problems. Works under moderate supervision. • Conducts psychosocial assessments and living conditions of patients and develops treatment recommendations. • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient. • Transports and utilizes required medical equipment and supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient homes/care facilities, VNS Health offices and other locations. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • Provides nursing care in accordance with Agency policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care. • Provides any of the following services to patients: advocacy, psycho-education, cognitive/behavioral interventions, monitoring of mental status, emotional support, brief psychotherapy, family based interventions. • Provides crisis intervention as necessary. • Consults with Program Coordinator as necessary to develop psychiatric evaluation and treatment plan. • Consults with referrer on assessment, recommendations, and update on progress of treatment. • Provides education, consultation and support to VNS Health staff. • Assist the health care provider in implementing the plan of care. • Follows a treatment plan, revising and updating as necessary. • Makes appropriate referrals for mental health services. • Participates in interdisciplinary team meetings, case conferences and clinical supervision. • Attends in-service education programs. • Assumes responsibility for continued professional growth. • Maintains productivity sufficient to meet VNS Health Home Care standards. • Maintains records in accordance with AMH and VNS Health record keeping standard and performs established workflow procedures. • Participates in special projects and performs other duties as assigned. Job Description The Department of Ambulatory Care Services at SUNY Downstate Health Sciences University is seeking a full-time Assistant Director of Nursing / Nurse Practitioner, Suite D Pediatrics. The successful candidate will: Provide primary care services to individuals and families; this will include care for incumbents own panel of patients, as well as seeing walk-in patients of the practice as needed. Document all care provided in the patients medical records. Develop further, maintain/manage patient care panel with participation in managed care programs and other direct patient care programs as applicable. On a daily bases, review all laboratory and radiology reports to identify abnormal results for urgent follow up; ensure that patients are contacted to arrange additional care and that their primary providers are notified.
