Contributes to Quality Improvement activities and participates in the departmental Quality Improvement Program. Responsible for own professional development to insure appropriate knowledge of advances in practice to provide state of the art treatment for patient population serviced. Perform other related duties as required. Requirements Licensed Registered Nurse in the State of Connecticut Medical/Surgical procedures experience required. Intravenous therapy experience preferred (included starting IV's) Good communication skills. We are seeking a Field Nurse Case Manager to cover Hartford and surrounding communities to include but not limited to New Britain, Bristol, Manchester, Glastonbury, and Springfield, MA. A Field Case Manager role is a field / home-based position with travel, up to 2 hours one-way (4 hours round trip), to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers. Staff Nurse-MS8-36hr

Under general supervision, assumes 24-hour accountability to plan and provide direct professional nursing care for specified patients throughout the entire episode of service delivery as defined by population and unit. Provides nursing services to patients and families in accordance with the standards of practice of a professional nurse, Connecticut Children’s policies & procedures, Joint Commission, and State & federal regulations. Reports to Nurse Manager. Clinical Practice- On a daily basis, demonstrates clinical expertise and uses the nursing process to provide safe, appropriate patient care to a defined patient population according to CCMC and unit policies and standards. Population Specific- Demonstrates knowledge of the differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families. Unit Leadership- Delegates duties and tasks associated with patient care to clinical staff according to hospital policy and their scope of practice.
