The clinician will float to various health centers and will work under the supervision of the Center Manager and in collaboration with the health center care team, to provide direct patient care in accordance with PPSNE protocols both in person and on telehealth. While working Care Coordination Team (CCT) sessions, under the supervision of the CCT Manager, the clinician is responsible for managing follow-up of abnormal results, signing off on lab reports and providing clinical and clinician support to the CCT.
