They can assist with planning for doctor's appointments, other meetings and may organize transportation for them. PURPOSE AND SCOPE: The professional registered nurse Inpatient RN CAP 2 may be an entry level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from CAP 1. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, applicable contractual service agreements, state and federal regulations. May be asked to provide specialized nursing care instructions to hospital/facility staff as stipulated contractually. May be assigned to assist in an Outpatient facility on an as needed basis. May serve as a Preceptor to new employees. Required to complete CAP requirements to maintain or advance. Performs all other duties as assigned by Supervisor. Overview Kelemen 10 is a medical-surgical unit that cares for patients who have various needs and disease processes. Both North and South 10 have Telemetry capabilities for our patients. Each side houses 36 patients to a maximum of 72 patients. South 10 has a 12-bed Intermediate Care Unit embedded within the 36-bed unit. INCU has critically care-trained RNs who support the patient's needs and care for ventilated patients. South 10 also has 2 negative pressure isolation rooms with an anti-room that is used when needed for specific isolation needs. We do multidisciplinary rounds Monday – Friday. This involves the patient in their care and allows the team members to collaborate with the patient to give them the best possible care and appropriate discharge to home or other facilities. We have a pharmacist who has an office on the unit to assist the RNs when needed to help avoid delay of care to any patient. You will work collaboratively with the interdisciplinary hospice team to ensure continuity and quality of care in patients’ homes and hospice offices. Key Responsibilities: Assess and reassess nursing needs of hospice patients regularly. Provide hospice nursing services, treatments, and preventive procedures. Initiate nursing procedures relevant to patient care and safety. Observe and report changes in patients’ physical or emotional conditions to physicians and the interdisciplinary team. Educate, supervise, and counsel patients and their families regarding care. Supervise and train nursing service personnel including hospice aides. Develop and update patient/family care plans with the interdisciplinary team. Participate in team conferences and maintain required patient records. Follow hospice policies, maintain confidentiality, and comply with HIPAA. Participate in on-call coverage as needed. Maintain professional skills and knowledge through ongoing education.
