Now Hiring - Certified Home Health Aide (CHHA) This Certified Home Health Aide will be responsible for, but not limited to: Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respirations as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Ensures new mothers and families have the basic understanding of newborn care and resources available after discharge. Demonstrates knowledge of medications and the correct administration based on age of patient and their clinical condition. Demonstrates proper technique for assessing fundal height, and fundal massage. Demonstrates ability to assess lochia, perineum, breast, bowel, bladder and emotional status. Responsible for obtaining supporting information and completing required documentation related to medication reconciliation for use by the Attending MD. Provides assessment and planning for individualized patient care in collaboration with physicians and other disciplines. Assesses the normal stages of labor, delivery and recovery, as well as, identifies any deviation of the normal pattern and progress and takes appropriate action. Utilizes fetal monitoring equipment, external and internal (FSE and IUPC), and correctly interprets FHR patterns and initiates appropriate nursing interventions as needed. Assists in the stabilizing of high-risk patients and accompanies them during transfer to a level two or tertiary care center as appropriate. Communicates with physicians about changes in patients’ clinical condition. Safely administers and demonstrates knowledge of medications used in labor and delivery. Implements appropriate procedure for labor checks, inductions, augmentations, NSTs, external cephalic versions and epidurals. Able to articulate care for patients with fetal demise based on perinatal loss protocol and appropriately provides care when applicable. Performs a systematic and thorough newborn assessment based upon knowledge of expected adaptation to extrauterine life so that deviation can be recognized. Communicates appropriately and clearly with physicians concerning any changes in newborn condition and/or abnormal diagnostic test results. Performs neonatal resuscitation, assisting physician as necessary. Identifies newborn problems, initiates appropriate nursing interventions, and assists in the stabilization of a critical newborn. Provides care and support to mother in her chosen method of infant feeding so that learning is enhanced. Demonstrates to new mothers appropriate feeding techniques using literature and teaching videos as appropriate (frequency and symptoms of adequate intake). Provides teaching to new mother on newborn care based upon individual patient/family needs so that new mother is able to care for newborn upon discharge. Utilizes informative discussions, videos, handouts, one on ones, teaching class to individualize newborn teaching. Utilizes appropriate nursing interventions for infants experiencing difficulty adjusting to extrauterine life. Sets up phototherapy, monitors vital signs, including pulse oximetry, according to policy and procedure; demonstrates proper technique for administering CPAP via mask or RAM cannula Demonstrates knowledge of medications and their correct administration based on age of newborn and his/her clinical condition. Under the direction of the RN circulator as dictated by the standards in the Medicare Conditions of Participation for surgical services, assists surgical teams during operative procedures in scrubbing for all physicians in assigned services. Arranges and inventories sterile set up for operations. Assists in preparing moving and positioning patients. Performs sterile scrub procedures and related activities such as preparation and draping the patient and operative area. Passes instruments, sutures, sponges and supplies in accordance with hospital policy and procedure. Participates in the turnover of operating rooms. $3,000.00 Sign-on Bonus Job Description I. Major Responsibilities: 1. Demonstrates independent functioning and proficiency in the scrub role in all cases in assigned specialty on an ongoing basis. Scrubs for all physicians in assigned service area, providing high quality evidenced-based clinical care with minimal supervision. Promotes teamwork within the team by formally precepting co-workers. 2. Prepares the Operating Room for cases. Selects and checks case cart and preference cards for correct instrument pan(s), packs, sterile supplies and additional items needed for the case. 3. Anticipates need for special equipment and supplies before procedure and requests it from the circulating nurse. Sets up back table and Mayo tray with instruments and supplies according to the procedure and surgeon preference card. Performs surgical count with circulating nurse according to established policy. 4. Responds to needs of surgeon and other team members. Assists operative team and anticipates needs of the surgeon and/or assistants during cases including the need for special equipment and supplies. Remains constantly observant of the operative field, recognizing emergency situations and performs emergency procedures as directed. Utilizes a variety of OR equipment such as microscopes, CUSA, steam sterilizers, suction, compressed air/gas, etc. 5. Passes and tracks instruments, sutures, and supplies during procedures. Labels all medication and solutions on sterile field. Monitors sponges, needles and instruments throughout the case. Cares for specimens to prevent loss or damage. Uses caution when handling specimens to ensure proper anatomical identification. 6. Assists in completing procedures. Participates in closing counts at appropriate times, sponge and needle count at change of shift according to the count policy. Assists in the application of dressing and/or cast. Assists with transferring patient from OR table to stretcher/bed when appropriate. 7. Prepares operating room and supplies for subsequent cases. Communicates with others to facilitate the timely turnover of operating rooms. Assures all sharps from back table are disposed of properly in biohazard sharps receptacle. Participates in cleaning, decontamination and sterilizing instruments and equipment. Turns over back table and gathers appropriate supplies and equipment for subsequent cases. 8. Participates in the maintenance and rotation of stock for the department. 9. Participates in maintaining cleanliness of the department. 10. Serves as a preceptor to new staff in specified clinical area. Mentors students and/or staff as requested. 11. Follows the Code of Ethics and Business Conduct policy and actively contributes to maintaining a positive work environment for patients, patients’ families/friends, visitors, co-workers, medical staff and customer groups. Position Qualifications: License/Certification/Education: Required: 1. Successful completion of an accredited school of surgical technology having obtained the required surgical technologists’ certification as of the date of hire. 2. All Surgical Technologist Certified employees will have obtained a surgical technologist credential administered by a nationally recognized surgical technologist certifying body accredited by the National Commission for Certifying Agencies and recognized by the American College of Surgeons and the Association of Surgical Technologists. 3. Maintain current Basic Life Support (BLS) certification every two years. Maintains AST membership and required educational units. Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. Department-specific competencies, including age-specific competencies and their measurements, will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
