Overview Oversees, supervises, coordinates, facilitates, and supports the Hospice After Hours care delivery activities, including field staff supervision. Manages continuity of care to all Hospice patients and their families after normal operating hours. Responds to incoming calls from patients/families/caregivers, assesses and priorities patient needs based on policies and procedures and staff capacity, ensuring all emergent and urgent request are met timely. Addresses needs within context of patient's Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies, and procedures. Ensures workload assigned is appropriate to staff’s ability and capacity in order to provide quality and comprehensive services; provides clinical oversight of nurses through coordination and coordination, and feedback from staff and patients/caregivers. Maintains current knowledge of clinical standard practices and the use of VNS HEALTH technology, to ensure care delivered is of the high quality, while maintain cost efficiency. Assesses the physical, emotional, and medical needs of hospice patients and their caregivers through after-hours telephone consult. Provides support and direction to families in accordance with the patient’s Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies, and procedures. Takes referral information and triages for immediate response, dispatches the on-call nurse to do the evaluation/ admission. Contacts Physician for verification of prognosis and appropriateness for Hospice/Palliative care. Contacts and interface with team members, primary physician, team physician, administrator, and other staff to ensure patient’s and family’s needs are met. Works directly with outside vendors and care delivery partners, to ensure when patients are transferred to General Inpatient Facilities, Emergency Departments, and Paramedicine Activation, that documents and patient information to ensure continuity of care and most effective care based on patient/caregiver goals and clinical needs. Provides clinical directions to patients/caregivers in accordance with Hospice guidelines. Obtains and receives verbal orders from team/physician and documents care using standard form. Evaluates and documents effectiveness of directions through telephone follow-up with patients/caregivers. Provides team/physician with documentation and implements any changes, as directed. Obtains and reviews daily report from each team on significant patient care situations that may require contact/follow-up during course of shift. Documents all calls received using high degree of accuracy and detail to ensure team/physician is provided with all relevant and necessary information. Completes case conference form on all calls received and faxes to respective team. Supervises on-call Nurses: Monitors/evaluates performance, provides guidance, recommends hiring and termination, as needed. Determines/Deploys on-call Nurse for home visits, provides necessary information and follows-up regarding status of visit and to obtain necessary report. Provides verbal report on significant events, plan of care changes and other patient/caregiver issues requiring further action and/or follow-up to each respective team. Participates in Interdisciplinary Hospice Team meetings, management meets and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training. Understands/implements/complies with Hospice Care Mission, objectives, policies, and procedures to ensure Hospice Care/patient/caregiver needs are met. Participates in special projects and performs other duties as assigned. Description Medical Office Specialist (Medical Assistant)-Primary Care/Teaching-New York Eye and Ear Infirmary of Mount Sinai-Full Time, Days, 9:45am to 6pm, M-F, Possible Weekends The Medical Office Specialist offers clinical and medical office support to physicians and surgeons. Assists in patient care and administrative duties necessary to deliver quality patient care. Performs various duties as a member of the health team in the treatment and care of patient and their families and also assists with tasks to support the unit. Participates and supports the health team in the direct patient care under the supervision of an RN/LPN or MD. Responsibilities Monitors patient flow in waiting room and resolves patient flow issues related to registration and post-visit processing Provides set-up of examination room and instruments in accordance with proper techniques established by hospital and infection control policies. Description Position Title: Licensed Practical Nurse-Institute for Advanced Medicine-Mount Sinai West-Full Time-Days Position Summary: Participates in the application of the nursing process to an assigned group of patients. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse. Responsibilities A. Clinical/Technical/Service: Participates in the application of the nursing process to an assigned group of patients. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse Performs job duties under supervision of a Registered Professional Nurse. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. Administer IV fluids, provided completion of annual competency IV therapy training. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. Makes recommendations in the application of the nursing process. Participates in planning, and administers direct patient care, including activities of daily living (ADL). Performs point of care testing including blood glucose. Performs nursing procedures as defined in the nursing procedure manual. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses. LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Description The Medical Assistant II provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Description As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities: Responsible for seeing that approved standards of resident care are maintained throughout the facility and overseeing the quality of nursing care. Delegates responsibility for direct resident care activities to Head Nurse/Nurse in Charge of each unit. Responsible for supervising, evaluating, counseling, and coaching care members on assigned units. Responsible for identifying and resolving problems at the unit level. Keeps the Director of Nursing/Assistant Director of Nursing informed of resident’s status through verbal communication and written reports. Makes daily rounds, visits every floor of responsibility, and checks acutely ill residents, admissions, and follows up on residents. Performs periodic audit of charting and documentation. Assures proper execution of physician's orders. Deals effectively, professionally, and tactfully with resident and family concerns. Submits weekly and monthly reports on statistics, meetings, and other pertinent information. Attends continuing education classes, seminars, workshops to maintain professional competency.
