Collaborate closely with physicians, nurses, social workers, chaplains, care managers, hospice teams, and community partners. Coordinate seamless transitions across care settings, including long-term care, hospice, home care, and hospital environments. Provide education and support to patients, families, caregivers, and healthcare professionals. Maintain accurate, timely clinical documentation and ensure compliance with regulatory and billing requirements. Participate in quality improvement initiatives, program development, and interdisciplinary team activities.
