What You Will Do: Performs home care visits after the initial assessment, collects on going data, and reports to the RN Clinical Supervisor, implementing the POC. Provides skilled nursing care in the patients home as ordered by the attending physician in conjunction with the Plan of Care. Autonomy within your position, use your training, critical thinking and knowledge of your patients to make decisions Submits clear and appropriate documentation in a timely manner and documents patient education. Communicates effectively to physicians, supervisors, and co-workers in a timely manner. Promotes continuity of care with appropriate admissions, transfers, and charges. Other duties as assigned. Position Summary: The nurse practitioner (NP) works in collaboration with a multidisciplinary health care team. The NP is generally service/physician aligned, managing a distinct patient population in the outpatient setting. The NP may be required to perform some coordination of inpatient care and facilitate discharges/outpatient follow up.
