About the unit We are a 64 bed Inpatient Rehab Unit that sees a variety of patients who need to work on both their functional and medical problems. We treat ortho patients recovering from hip fractures, complex knee replacement, back surgery with neurologic involvement, multi-trauma, and other orthopedic issues. We also see neurology patients recovering from stroke, brain and spinal cord tumors, Traumatic Brain Injuries, Parkinson’s, MS, and any cancer diagnosis with functional decline. We also have patients come to our unit who had amputations and other complex medical/surgical issues with functional decline. Patients participate in 15 hours of therapy across the 7-day week. Nursing continues the interventions practiced by the patients in therapy sessions, to continue safe mobility. For many of our patients, Rehab is the key to getting back home with their families. This is a very rewarding unit to work on, as we all get to take part in the progress patients make towards their goal of getting home. Duties and responsibilities may include and not be limited to: Performing patient assessments, interviewing patients about current health problems, medication history Taking vital signs including blood pressure, temperature, and weight Providing wound care, including cleaning and bandaging injured areas Administering medication, including injections of medications and monitoring reaction to meds Maintaining medical records, including entering information into our electronic health record, TechCare Observing patient health and mental health status Providing emergency first aid Providing patient education and discharge/release planning Qualifications Experience / Requirements / Qualifications Graduate from a Licensed Practical Nursing program High school diploma or equivalent (GED) Actively state-certified as a Licensed Vocational Nurse or Licensed Practical Nurse Must possess an active BLS CPR Certification (American Heart Association preferred) Able to maintain confidentiality of all proprietary and/or confidential information Display integrity, professionalism and be able to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements Must pass the facility’s criminal background check and drug screening Physical Capabilities: Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift. Clinical RN Coordinator - Detroit

SUMMARY: The Clinic Registered Nurse Coordinator serves in the role of Clinical Coordinator responsible for ensuring the functions optimally by appropriately delegating and monitoring the activities of the Licensed Practical Nurse, Medical Assistant and Scheduling Clerks. Utilizes a systematic approach to nursing practice which incorporates all aspects of the nursing process including, assessment, planning, implementation and evaluation of PACE Participants. The Clinic RN Coordinator demonstrates a direct relationship between nursing interventions and participant outcomes, demonstrates clinical competence and engages in effective patient teaching for acute and chronic illnesses. The focus of care is one that enhances functional capacity, encourages autonomy in all aspects of care, and encourages Participant, Caregiver and Family involvement in achieving health care goals. SPECIFIC DUTIES AND FUNCTIONS: Collaboration with the Physician and Nurse Practitioner to ensure optimal clinical flow. Daily workflow of clinical activities and delegation of task to the clinic Licensed Practical Nurse, Medical Assistant and Scheduling Clerk. Triaging and assessment of Participants in the clinic setting and providing education resources. Supervises the LPN and Medical Assistants to ensure competency of clinical skills and task. Collaborates with Manager to ensure adherence to quality initiatives in the clinic setting and organizational. Ensures that clinical activities are documented in the Participants medical record by clinic staff daily. Ensures that documentation of clinical activities are evidence-based and adheres to standards set by quality and professional standards. Collaborates with RN Case Managers regarding care delivered to Participants in the clinic and any transitions of care, through verbal and electronic communication. Collaborates with all Interdisciplinary Team members regarding care delivered to Participants in the clinic and any transitions of care, through verbal and electronic communication. Collaborates with Nurse Educators regarding any assessed educational needs of staff and new hire competency skills compliance. KNOWLEDGE, SKILLS AND ABILITIES: BSN preferred. At least one year working with the frail elderly population. Five years of management experience preferred. Must have the ability to lead and direct staff in the delivery of care. Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact. Excellent oral and written communication skills necessary; must have strong interpersonal skills. Must have the ability to understand and standardize process related to quality indicators and utilize technology to document and present data.
