IMT teams deliver care directly in the community, including shelters, public spaces, and other natural settings. Position Overview The Registered Nurse (RN) serves as a primary practitioner responsible for assessing and monitoring participants’ clinical health and response to treatment. The RN works collaboratively with a multidisciplinary team including psychiatry, social work, peer specialists, and other staff to support participants in achieving their mental health and recovery goals. Key Responsibilities • Conduct psychiatric and physical health assessments for program participants. • Develop and coordinate care plans with the multidisciplinary treatment team. • Monitor participant health status and modify treatment plans as needed. • Administer medications, injections, vaccines, and perform phlebotomy as ordered. • Monitor vital signs and medication side effects and communicate findings to the treatment team. • Coordinate medical appointments and collaborate with psychiatrists and community physicians. • Provide treatment, rehabilitation, and recovery support services. • Advocate for participants and assist them in accessing community resources and services. • Maintain accurate clinical documentation, progress notes, and medical records. • Participate in team meetings, supervision, and required trainings. • Provide crisis intervention services on a rotating basis.
