Assists in escorting patients. Ensure chart is available, with relevant lab work and test reports if indicated, for physician when the physician sees the patient. • Maintains an awareness of current patient centered evidence-based practices and uses knowledge of applicable research data in provision of patient care. • Collaborates with members of the interdisciplinary team and promotes collegial relationships. • Assists patients and communicates with licensed providers to obtain prescription renewals as needed. • Performs patient related functions as directed by the Licensed Provider and/or Registered Nurse (RN), and documents properly. These may include the following, as governed by LPN scope of practice: medication administration, vaccination, other injections, and patch placement, set up and/or assist for procedure. • Communicates directly with patients regarding their care or treatment using appropriate terminology and respectful manner. Interacts with medical staff efficiently.
